Method of early diagnostics and correction of temporomandibular joint dysfunctions Method of early diagnostics and correction of temporomandibular joint dysfunctions / 2445043
Invention relates to medicine, namely to dentistry and is intended for correction of detected by early diagnostics temporomandibular joint (TMJ) dysfunctions. Anthropometry of face is carried out in order to determine displacement of lower jaw and measure the height of lower third part of face. Displacement of lower jaw is determined in sagittal, transversal and distal directions. In order to determine distal displacement of lower jaw Eshler-Bittner test is applied. To determine lateral displacements occlusography is carried out in habitual occlusion, results are obtained by computer program T-Scan. Orthopantomography is performed and value of joint spaces is determined. If secondary displacements of lower law in sagittal and/or transversal and/or distal directions are determined, joint spaces of various value on orthopantomogram and change of height of lower third part of face are present, dysfunction of TMJ is diagnosed. Lower jaw is installed in neutral state in central occlusion, mouthguard with bite sites for fixation of lower jaw in said position is made. Occlusography is repeated in central occlusion together with mouthguard, difference of initial and blocking contacts is determined and in case if contact is advanced, selective lapping is carried out. Conclusion about elimination of dysfunction is made on the basis of patient's subjective sensations, anthropometric data and holding of lower jaw in position without displacement in sagittal and transversal planes. Final correction of mouthguard is performed in central occlusion under control of OPTG with central position of TMJ head in glenoid fossa, after which mouthguard from silicon is replaced with plastic. Myotherapy is administered and orthodontic treatment and/or dental prosthetics is performed after adaptation in accordance with indications.

The invention relates to medicine, and more particularly to orthodontic devices, and is intended for treatment of distal occlusion caused ucor">

Distal occlusion is one of the most common dental anomalies in children and adults, a long-term treatment is often accompanied by relapses, which many experts attribute to insufficient rebuilding the muscles of the floor of mouth, cheeks and lips.

Known duchaussoy frame apparatus MMSI for the treatment of distal occlusion of the dentition (1), contains the basis of the maxillary plate with vestibular arch and clasps of Adams on the first permanent molars, niggunim bandage, steel wire frame that holds the lower jaw in its extended position. This device has a number of disadvantages:
- the wire frame and pilot often break at the place of exit from the base of the maxillary plates,
- the wire frame that attaches the lower jaw, not to be activated,
- during sleep by relaxing the muscles of the lower jaw drops, taken backwards and not kept on a wire frame that extends the duration of treatment,
- structural features of the apparatus does not allow the extension of dentition,
- the device has no effect on the buccal musculature.

To some extent, these shortcomings can be attributed to orthodontists ensuring the efficiency of treatment of distal occlusion, normalize the size of the dentition and the lower jaw position.

The problem is solved in that the device for orthodontic treatment that contains the base plate on the lower and upper jaw, which are interconnected in a constructive occlusion by means of the fixing device, pilot for the lower lip and buccal pelota, unlike closest analogue further comprises the clasps of Adams and orthodontic screws. This fixture includes fixed on the maxillary plate guide tube for entry of l-shaped pins that are installed on the lower plate, on which are fixed pilot for the lower lip, and cheek pelota elastically connected by an elastic thrust.

Given a set of essential features provides technical result in all cases to which the requested amount of legal protection.

In special cases run, the device characterized by the fact that his orthodontic screws welded in sagittal sections of the base plates, and the fact that vestibular arc is located on the maxillary plate.

the STI mouth, the maxillary part of the device of Fig. 4 is a view from the side of the mouth, the jaw portion of the device.

Device for the treatment of distal occlusion of the dentition contains two interconnected in a constructive occlusion removable base plates 1 and 2, respectively on the upper and lower jaw, vestibular arc 3 to eliminate the protrusion of the upper front teeth, clasps of Adams on the upper first permanent molars 4, toggle clasps 5 to the upper and lower cheek peloton 6, 7 are welded into them hooks 8, 9, on which is mounted the connecting ring elastic rod 10, the plastic vestibular plot 11, tailrace lower lip from the alveolar ridge, steel tube 12 with a diameter of 1.4 mm, welded to the base plate 1 along the upper premolars, l-shaped wire pins 13 with a diameter of 1.2 mm, fixed in the lower plate 2 and are included in the tube 12, the locking vzaimoprityazhenie upper and lower plates. If indicated (narrowing of the dental arches and other ) warialda orthodontic extend the screws 14 and (or) the front arc 15.

The proposed device for the treatment of distal occlusion captures the dentitions in a constructive occlusion by leasti mouth.

Between pearlite colonies wire-pipe connection 13, 12 helps to keep the lower jaw in a constructive occlusion and, if necessary, to activate the l-shaped pins 13. Vestibular arc 3 allows to reject the upper front teeth and enhances reciprocal load on the lower dentition. Lip and cheek pelota 11, 6, 7 drains the muscles of the cheeks and lower lip from the apical base of the lower dentition and the upper dentition in the posterior teeth, normalizes geodinamicheskogo equilibrium in the maxillofacial region. The hooks 8, 9 and stretchy elastic rod 10 hold of pelota in the desired position. Orthodontic screws 14 allow to extend the teeth. Face-bow 15 allows the sagittal and vertical alignment of the upper dentition.

The use of this device enables a metered movement of the lower jaw, with readings to expand the dentitions, eliminates protrusions the inclination of the upper front teeth, contributes to the shortening of the upper dentition and the lengthening of the lower dentition, eliminates the sagittal slit provides good fixing apparatus, increases the effectiveness of treatment of distal occlusion reduces its terms.
