SUBSTANCE: invention relates to field of medicine, namely to orthopedic dentistry and can be used in manufacturing of clasp telescopic prostheses with additional lock fastening. Method of manufacturing of additional lock fastening for clasp telescopic prosthesis includes several stages. At the stage of modelling wax reproduction of internal telescopic crowns on their distal surfaces performed is installation of additional lock fastenings in form of industrial workpieces of attachment with preservation of diametrical location of support teeth. Casting from metal of internal telescopic crowns with spherical projections is carried out. Fixation of medium rigidity matrices is performed on spherical projection of internal telescopic crown, model is prepared for process of duplication with isolation of clearance between lower matrix surface and mucosa of prosthesis bed. Wax modelling of framework of clasp telescopic prosthesis with external telescopic crowns on fireproof model is carried out. After casting framework of clasp prosthesis by traditional method, inside framework of detachable part of clasp prosthesis socket for installation of matrices under spherical projection is formed. After casting framework of clasp prosthesis by traditional method, fitting of metal construction without matrices is performed.

EFFECT: increased quality of prosthetics due to application of additional lock fastening, with possibility of its re-activation and ensuring prolongation of prosthesis service terms, without essential change of traditional process of manufacturing of telescopic clasp prostheses.

Today in the clinic of orthopedic dentistry prosthetics partial defects of dentition are different kinds of orthopedic structures in combination with a variety of anchoring systems, including telescopic. Negative aspect of using this fixation systems is the difficulty of providing adequate fixation of the prosthesis with a significant taper abutment crowns, as well as the recovery of lost traction due to gradual wear of the metal telescopic crowns.

The known method of applying telescopic fixation systems clasp dentures in combination with an additional locking bracket. This castle mount comprises a cylindrical housing and a part of him locking rod. The rod is made of metal and provided with a support shoulder and zaovalennymi in the form of hemispheres working end. On the body of the rod in pojetom condition worn spring, based on zahlcovany in the housing area. The mount is made integral. The outer surface of the housing is fitted with a retention thread, with which the castle mount is fixed in the frame clasp prosthesis. The disadvantages of using this type of telescopic systems fixing clasp dentures in combination with an additional locking bracket are: the inability to re-activate additional locking fasteners in connection with the gradual reduction in the fastening force caused by the abrasion of the metal surface of the working part of the piston rod and the inner surface of the telescope, reducing the fixation of the prosthesis due to the gradual reduction of the pressure of the spring on the locking rod, the complexity of manufacturing such structures.

In this regard, the most important task is creating a new type of clasp telescopic prosthesis additional locking mount that allows you to initially increase the term of functioning of the prosthesis.

The technical result of the claimed method is to improve the quality of the prosthesis through the use of additional locking clasp fastening to the telescopic prostheses in the form of attachments spherical type, with the possibility of re-activation and security cont the possible service life of the prosthesis without a significant change in the traditional manufacturing process telescopic clasp dentures and increase financial and technical production costs.

The technical result is achieved by the fact that at the stage of modeling wax reproductions of the inner telescopic crowns installs using the template on the distal surfaces of the inner telescopic crowns subject diametrically location of the abutment teeth, additional lock fastenings in the form of industrial stocks of attachmen VSC-SG-type (company Bredent) (Fig.1). Next, make a casting of the metal inner telescopic crowns with spherical protrusions (figure 2). This was followed by a fixation matrices of medium hardness on the spherical projection of the inner telescopic crowns and prepare the model to the process of duplication with the insulation gap between the bottom surface of the matrix and the mucous membrane of prosthetic bed. Then carry out a wax modelling framework clasp telescopic prosthesis with external telescopic crowns on the refractory model. After casting frame clasp denture in the traditional way are fitting metal structures without matrix.

The activation of the additional locking fasteners by pressing matrices in the nest is made only after a significant loss of adhesion forces of telescopic crowns, as well as in insufficient fixation of the prosthesis due to significant taper supporting the teeth upon delivery of the work. Depending on the need to provide different degrees of fixation clasp prosthesis may use standard matrix parts VSC-SG-type with different markings (figure 3). Installing a replaceable matrix parts is not particularly difficult and does not require additional materials and technical means.

Given the above, the proposed differences meet the requirement of "novelty" and "inventive step".

An example of the application of this kind of structures are presented in the clinical case of a patient Century, 46 years old, attending 4 years ago with a diagnosis of partial secondary edentulous upper jaw I class 1 subclass by Kennedy. The patient was made telescopic clasp denture in the upper jaw on the proposed technology with the fixation on 1.3, 1.4, 2.3 teeth (figure 4). After 4 years, the patient noted a decrease in fixation strength and the appearance of a vertical mikroekskursy telescopic clasp prosthesis relative to the prosthetic bed. To eliminate the above complaints were pressing the standard matrices of spherical attachments VSC-SG-type with an average degree of stiffness in a pre-formed socket (Fig.5). After fitting the prosthesis in the mouth with activated additional locking bracket he was fixed on PR is asnom bed (Fig.6).

To determine the degree of reliability of the proposed method, we performed orthopedic treatment group patients from 17 people who were made 24 telescopic clasp prosthesis on the proposed technology.

Observations allow us to conclude that the claimed invention meets the requirement of "industrial applicability".

Advantages of the claimed method. The use of the method of manufacturing clasp telescopic prosthesis additional locking bracket allows you to:

• originally to extend the period of operation of the telescopic clasp dentures;

• to ensure the provision of adequate fixation of the prosthesis in clinical conditions with a significant taper of the abutment teeth.

Thus, in General, allows to improve the quality of prosthetics.

A clasp system for fixing a removable partial denture of a single tooth consists of a basis with artificial teeth and a denture clasp. One end of the clasp comprises a mechanical device consisting of an inner O-ring abutment and an outer matrix and configured as a pivot, while the other end comprises a clasp seat in a rigid socket on an artificial dental crown. The outer matrix is fixed in the prosthesis basis and detachable together with the prosthesis, while the inner O-ring abutment is ball-shaped and connected to a clasp arm. The seat of the clasp socket is mounted on the artificial dental crown vertically or horizontally either from the lingual, or palatal, or approximal surface.

EFFECT: invention enables providing a more stable fixation of the detachable clasp system, a masticatory effectiveness and an appearance, and also avoiding the negative effect on the parodontium.
