SUBSTANCE: invention relates to a composition for alimentation of babies with low, very low or extremely low body weight at birth as well we for alimentation of premature babies. The composition includes 100-200 mg of threonine per 100 kcal, 5-15 wt % of palmitic acid of the total fatty acids content and prebiotic oligosaccharides capable to reduce pH in the digestive tract (DT). Additionally the composition contains 2 - 25 wt % of medium-chain fatty acids of the total fatty acids content. The composition may contain an undigestible oligosaccharide represented by galactose.

EFFECT: invention allows to enhance oral tolerance, reduce duration of food transit through DT, increase bowels movement periodicity, stimulate the immune system, accelerate initiation of complete enteroalimentation of babies born before 37 pregnancy weeks and/or babies with weight at birth less than 2500 g.

The present invention relates to a method of feeding infants with low birth weight (LBW children), children with very low birth weight (VLBW children), children with extremely low birth weight (ELBW children) and preterm infants to improve oral tolerance.

Because of the underdevelopment of the digestive tract of children born prematurely, suffer from problems associated with the digestive tract such as constipation, difficulty swallowing, gagging, belching, cramps and digestive problems. In addition, often they are not tolerant to oral nutrition. As a consequence, often premature infants fed parenterally in a long period that is associated with a high risk of secondary complications such as infection and atrophy of the intestine. Therefore, it is desirable to reduce the duration of parenteral feeding and promote oral tolerance.

There are many mixtures for normal term infants. However, they are not optimal for the introduction of weak children, such as preterm infants. As a result, the disadvantages of this mixture appear in the worst digestibility of feeding preterm children.

In EP 0758846 describes essentially containing the fat composition used in high-grade mixtures in which palmitic impacilla and oleic acid, contained in the oil, interchangeable. Additionally, the invention includes interchangeable composition with the introduced medium chain triglycerides, in particular, for use in nutritional products for premature infants or children with low birth weight.

In WO 96/29881 describes food for premature babies and babies and diet foods based on milk, milk substitutes and milk products of mammals, along with the way they are received. Foods contain in the form of monomers, some or all of albumin and beta-lactoglobulin. The ratio of alpha-lactalbumin beta-lactoglobulin improved compared with milk of ruminants. Foods contain at least the same amount of beta-casein, as alpha s-casein.

The present invention relates to a method of feeding infants with low birth weight (LBW children), children with very low birth weight (VLBW children with extremely low birth weight (ELBW children) and preterm (premature babies) children (here and below under the generic term "poor children") mixture, in particular, adapted for stimulation of tolerance to the power of weak children. The weak children underdeveloped gastrointestinal tract and, therefore, often there are problems with a chair. In particular, increased time of passage is the cause of constipation and also suppresses oral tolerance to food composition. Therefore, the present invention can be used to stimulate oral tolerance and/or reduce problems with chair and/or reduce the risk of infections in children with weak.

It is established that precipitates the salts of palmitic acid are the main cause of constipation in premature infants breastfed baby food. The present composition is particularly suitable for preventing the formation of precipitates of calcium. Additionally, the present composition dissolution of precipitates stimulates the development of the gastrointestinal tract and improves its barrier function.

The present invention relates to compositions suitable for feeding weak children where taken several measures to overcome the above described problems. Preferably undertaken the following measures:

- reduced the content of palmitic acid in the nutrient composition that results in a reduction in the formation of calcium Soaps of palmitic, which makes the stools firmer,

In addition to the above-mentioned effect of reducing the formation of Soaps, as a result of the decrease in the content of palmitic acid, the presence of GAL-oligo also leads to the increase of bioavailability of calcium. Therefore, the present invention also relates to a method of stimulating the absorption of calcium from the weak children.

To obtain a balanced and optimal for absorption in preterm children of amino acids with the aim of improving the development of intestinal and oral tolerance, it is preferable to use a protein with a low content of threonine. Suitable protein sources, for example, are acidic whey and/or sweet whey with low content of glycomacropeptide (GMP).

By reducing the content of palmitic acid and the inclusion in the composition (low-calorie or low-calorie) prebiotic oligosaccharides (preferably GAL-oligo) it is important to maintain optimal caloric value and osmolarity. Prebiotic fiber according to the invention has contributed to the improvement of the barrier function of the intestines by improving the quality and quantity of mucous. Also, the authors of the present invention have found that low concentrations of long-chain polyunsaturated fatty acids (LC PUFA) effectively stimulate the integrity of the intestinal barrier and/or reduce the permeability of the intestine. Therefore, to further improve the integrity of the intestinal preferably LC PUFA. LC PUFA reduce the permeability of the gastrointestinal tract. The combination of GAL-oligo and LC PUFA synergistically improves barrier integrity.

The predominant effects of the present invention include, for example, reduce the time of passage and the risk reduction is of Uporov, improving oral tolerance weak children. In one aspect the present invention relates to the use of a composition containing threonine from 100 to 200 mg per 100 kcal, comprising: a) from 5 to 15 wt.% palmitic acid of the total fatty acids and (b) prebiotic oligosaccharides capable of lowering the pH in the gastrointestinal tract, to obtain a nutritional composition that reduces the time that can be started full enteral nutrition and/or treatment and/or prophylaxis of infections in premature infants, children with low birth weight (LBW children), children with very low birth weight (VLBW children and/or infants with extremely low birth weight (ELBW children).

In an additional aspect, the present invention relates to compositions suitable for the introduction of premature children, children with low birth weight (LBW children), children with very low birth weight (VLBW children and/or children with extremely low birth weight (ELBW children), compositions containing threonine from 100 to 200 mg per 100 kcal, comprising: a) from 5 to 15 wt.% palmitic acid of the total fatty acids and (b)prebiotic oligosaccharides.

Children

The present invention relates to a method for feeding selected from the group consisting of children with low birth weight (LBW children), children with very low birth weight (VLBW children and/or infants with extremely low birth weight (ELBW children) and preterm (premature babies) children, here and further generalized by the term "poor children". Children with low birth weight have a birth weight less than 2500 grams. Children with very low birth weight have a birth weight less than 1500 grams. Children with extremely low birth weight have a birth weight less than 1000 grams. Premature (or prematurely born children are babies born before 37 weeks of pregnancy.

Application

In one aspect the present invention relates to a method of treatment and/or prevention of disorders in weak children. In an additional aspect, the present invention relates to a method for feeding weak children, including the introduction of the specified weak children of the composition of the present invention. With the introduction of the compositions of the present invention decreases the risk of constipation due to the reduced time of passage of food through the gastrointestinal tract, softens the stool and increases the frequency of stools. This results in a reduction in the occurrence of spasms in the stomach and ASECNA tract and reduce the risk of infections. As a consequence, improvement of function of the gastrointestinal tract leading to improved oral tolerance by feeding the child and decrease start time to full enteral feeding. This reduces the risk of infection and atrophy of the intestine with parenteral feeding.

Therefore, the present invention relates to a method, providing for vulnerable children:

a) treatment and/or prevention of constipation;

b) reducing the time of passage of food through the gastrointestinal tract;

c) an increase in the frequency of stool;

d) the softening of the stool;

e) the treatment and/or prevention of intestinal spasms;

f) treatment and/or prevention of infections;

g) strengthening of oral tolerance;

h) the stimulation of the immune system and/or

i) the decrease start time to full enteral feeding.

In one embodiment, the present invention relates to a method of treatment and/or prophylaxis of infections selected from the group consisting of necrotizing enterocolitis, infection of the gastrointestinal tract, respiratory tract infections, and infections of the urinary tract, in particular, the introduction of the specified weak children of the composition of the present invention.

Palmitic acid

Palmitic acid (16:0) is a very important nutrient in the diet of weak children. However, the normal content palmit the new acid in the diet for children with weak too high. The high content of palmitic acid causes the formation of Soaps, constipation, and some other negative side effects. Therefore, the composition of the present invention contains from 5 to 15 wt.% palmitic acid of the total fatty acids, more preferably palmitic acid included in the composition in the form of triglycerides.

Oligosaccharide

The composition used in the method according to the present invention preferably contains prebiotic oligosaccharides capable of lowering the pH in the gastrointestinal tract. Used in the present invention, the term "oligosaccharide" refers to saccharides with a degree of polymerization (SP) sharidny units greater than 2, which remain undigested or only partially digested in the intestine under the influence of acids or digestive enzymes of the upper section of a human's digestive tract (small intestine and stomach), but which also are fermented by intestinal flora of man. The oligosaccharides used in the present invention, preferably are saccharides with a degree of polymerization of preferably below 60 sharidny units, more preferably below 40 sharidny units, even more preferably below 20 sharidny units, most preferably below 10 sharidny units.

Predpochtite the flax oligosaccharides according to the present invention are water-soluble. The solubility may be determined appropriate by the method described L. Prosky et al, J. Assoc. Anal. Chem 71 : 1017-1023, 1988. Used herein, the term "parenterally" refers to the ability to be cleaved by microorganisms in the lower part of the gastrointestinal tract (e.g., the colon) to smaller molecules, in particular conversion to short-chain fatty acids and lactate. In a more preferred embodiment of the present invention, at least 80 wt.% the oligosaccharides of the present invention are prebiotics. "Prebiotics" are defined as nevereverever ingredients of the food product, which selectively stimulate the growth and/or activity of one or more bacterial species in the colon and, therefore, have a beneficial effect on the host (Gibson and Roberfroid, J. Nutr. 125: 1401 - 1412(1995)). Preferably prebiotic oligosaccharides of the present invention are selected from the group consisting of fructooligosacharides (e.g., inulin, fructo-oligosaccharides, are not digestible dextrins, galactooligosaccharides (including transplantological), xylooligosaccharides, arabinopyranoside, glucooligosaccharides, mannooligosaccharides, and mixtures thereof. Preferred oligosaccharides according to the invention are selected from the group consisting of fructooligosaccharides, fructo-oligosaccharides and galacto is oligosacharides. The preferred oligosaccharide according to the invention represents a galactose containing nevereverever oligosaccharide.

Galactose containing oligosaccharides

Found that galactose containing nevereverever oligosaccharide (here and hereinafter referred to as "GAL-oligo"), effective to reduce the pH of the intestine and/or stimulation of the formation of lactate in the digestive tract (see Example 1). This ensures a reduction in the formation of Soaps and/or stimulates the dissolution of the precipitated Soaps. Therefore, the composition according to the invention contains galactose containing nevereverever oligosaccharide.

Galactose of the present invention, containing nevereverever oligosaccharide (GAL-oligo), preferably contains at least two terminal sacharine units, where at least one terminal sahariana unit selected from the group consisting of glucose and galactose; and at least one terminal sahariana unit selected from the group consisting of galactose and factoty. Preferably, at least 75% of the sugars GAL-oligo represent ß-linked, preferably 100%.

The term "terminal saccharide" refers to a saccharide, which is connected with one another sacharides unit (e.g., galactose, glucose, fructose, or fucose). GAL-oligo according to the present invention preferably contain no more than the eat 4 of the terminal saccharide, preferably not more than 2. In a preferred embodiment of the invention GAL-oligo contain at least one terminal galactose and one of the selected at least one terminal glucose and one terminal fucose. Even more preferably GAL-oligo according to the invention include at least one terminal galactose and at least one terminal glucose. Preferably oligosaccharides containing terminal 2 sacharine units with a total sharidny units from 2 to 60.

Preferably GAL-oligo is selected from the group consisting of transplantationfollow, galactooligosaccharides, lacto-N-tetraone (LNT), lacto-N-neoteroi (neo-LNT), fucosyl-lactose, fokusirovannyi LNT and fokusirovannyi neo-LNT. In a particularly preferred variant of the invention the method includes the introduction of transplantationfollow ([galactose]n-glucose; where n is an integer from 1 to 60, that is,, 2, 3, 4, 5, 6, ...., 59, 60; preferably, n is selected from 2, 3, 4, 5, 6, 7, 8, 9 or 10). Transplantological (CBT) is available, for example, a trademark of VivinalTM(Borculo Domo Ingredients, Netherlands). The preferred sugars are transplantological associated ß-connection.

The composition according to the present invention preferably includes from 0.1 to 12 grams GAL-oligo per 100 grams dry weight of the composition, preferably the t 0.5 to 8 grams more preferably from 1.0 to 7.5 grams. After recovery of the powder by the liquid, and introducing the liquid mixture for child nutrition child these quantities GAL-oligo provide the desired effect without causing discomfort in the gastrointestinal tract.

Various oligosaccharides

For optimum lowering of the pH effect on the entire gastrointestinal tract, the composition preferably contains two different oligosaccharide. Each oligosaccharide fermented by various microorganisms of the intestinal microflora and/or in different areas of the gastrointestinal tract that results in a decrease in pH, due to the enhanced formation of short-chain fatty acids (CCGC). Reduced pH and increased the formation CCGC reduces the formation of Soaps and reduces the risk of constipation.

The present invention relates to compositions, which preferably includes oligosaccharide a and B. oligosaccharide Oligosaccharide a and B oligosaccharide have different glycosidic bonds, varying degrees of polymerization and/or different monosaccharide composition. According to a preferred variant of the invention, the percentage of at least one monosaccharide selected from the group consisting of glucose, fructose is galactose oligosaccharide A, is at least 40% higher than the percentage of the same monosaccharide in B oligosaccharide, preferably at least 50%, more preferably at least 75%, even more preferably at least 90%. High diversinet monosaccharides encourages broader population of probiotic bacteria, resulting in reduced pH and increased stimulation of short-chain fatty acids. The percentage of monosaccharides in the saccharide can be easily calculated by dividing the number of relevant monosaccharide units (e.g., glucose) in the saccharide on the total number of monosaccharide units in the saccharide.

Preferably the oligosaccharide a and B oligosaccharide have a degree of polymerization (SP) from 2 to 200. Preferably, at least 80 wt.%, more preferably at least 95 wt.%, most preferably at least 98 wt.% the total weight of the oligosaccharide A and B has a degree of polymerization (SP) below 100, more preferably less than 60, most preferably below 40. Lower SP mainly reduces the viscosity and increases terminterest not digested sugars. Preferably, at least 50 wt.%, more preferably at least 75 wt.%, the total weight of the oligosaccharide A and B represent nevereverever sugars JV 2-9. Preferably the oligosaccharide has A JV 2-9, more preferably 2-8. Preferably the oligosaccharide B has a JV 10-100. The saccharides A and B with different SP can have the same or slightly different monosaccharide composition, preferably, different monosaccharide composition.

In a preferred embodiment of the invention, the percentage of at least one type of glycosidic linkages of A saccharide from all glycosidic linkages of A oligosaccharide, at least 40% higher% same glycosidic bonds in B oligosaccharide, preferably at least 50%, more preferably at least 75%. Used in the present invention, the term "glycosidic bond" refers to C-O-C bond formed between two rings of cyclic monosaccharides by the removal of water. Preferably glycosidic bonds in the oligosaccharide B include at least 40 % ß(2,l) glycosidic linkages, more preferably at least 75%.

In a preferred embodiment, the composition according to the invention contains a combination of galactooligosaccharide with an average SP from 2 to 10 and oligosaccharide selected from the group consisting of fructooligosacharides (e.g., inulin and fructo-oligosaccharides, more preferably a combination of galactooligosaccharides and inulin.

Protein

The composition according to the invention contains proteins. Proteins are essential for growth weak children. As a source of protein for weak children very suitable whey protein.

However, as a rule, the serum has a high content of threonine. The high content of threonine in the result can lead to hypertensie. Therefore, preferably taken to SN is the position of the content of threonine in the composition of the invention.

Thanks to the low content of threonine, the composition according to the invention preferably contains an acid whey or sweet whey from which it is removed at least part of glycomacropeptides (GMP). Preferably the protein component contains hydrolyzed intact protein, hydrolyzed protein and/or amino acids. Preferably the composition according to the present invention contains from 100 to 200 mg threonine per 100 kcal, preferably from 125 to 175 mg threonine per 100 kcal.

Medium chain fatty acids

Due to the low content of palmitic acid caloric value and fat content in the composition can be greatly reduced. However, this is undesirable. Therefore, in order to ensure sufficient intake of fat and calories, at least part of the fat fraction provided with medium chain triglycerides. Medium chain triglycerides have an advantage over long-chain fatty acids because they are more quickly and are absorbed in the gastrointestinal tract therefore reduces the formation of calcium Soaps and decreases the risk of constipation, which results to the improvement of oral tolerance. Medium chain fatty acids (MCT) include Caprylic acid (C8:0), capric acid(C10:0) and lauric acid (C 12:0). Normally, they are provided in the form of medium chain triglycerides (medium chain triglycerides (MCT), where SP refers to the length of the chain consisting of C8:0 C 10:0 and C12:0. Preferably the present invention contains from 2 to 50 wt.% medium chain fatty acids of the total weight of fatty acids, preferably from 2 to 25 wt.%, even more preferably from 2 to 10 wt.%. Since lauric acid still has the ability to form calcium soap, preferably the composition comprises less than 25 wt.% lauric acid to the total weight of fatty acids, preferably below 12 wt.%, more preferably below 10 wt.% the total weight of fatty acids.

Polyunsaturated fatty acids

Also, the authors of the present invention found that icosapentaenoic acid (EPA, n-3), docosahexaenoic acid (DHA, n-3) and arachidonic acid (AA, n-6) effectively reduce the permeability of the intestinal impenetrable walls. The authors of the present invention have found that oral tolerance is partly driven by increased development of the intestine and, therefore, leads to a decrease in the permeability of the intestine. Therefore, the composition according to the invention includes at least one long-chain polyunsaturated fatty acid, select the ing group, consisting of EPA, DHA and AA.

Low concentration of LC PUFA effectively reduces the permeability of the intestinal impenetrable walls. Therefore, the content of LC PUFA with 20 and 22 carbon atoms in the composition of the present invention does not exceed 10 wt.% of the total fat content preferably does not exceed 5 wt.%, even more preferably, not more than 3 wt.% of the total fat content. Preferably the composition according to the invention comprises at least 0.1 wt.%, preferably, at least 0.25 wt.%, more preferably, at least 0.5 wt.%, even more preferably at least 0.75 wt.% LC PUFA with 20 and 22 carbon atoms of the total fat content. Preferably the omega-3 LC PUFA does not exceed 1 wt.% of the total fat content; preferably the omega-6 LC PUFA does not exceed 2 wt.% of the total fat content; preferably the content of AA (omega-6) is below 1 wt.% of the total fat content; and/or preferably the weight ratio of EPA/DHA is 1 or below, more preferably below 0.5.

The EPA content preferably does not exceed 5 wt.% of the total fat content, more preferably less than 1 wt.%, but is preferably at least 0.1 wt.% of the total fat content. The DHA content preferably does not exceed 5 wt.% of the total fat content, more predpochtite the flax does not exceed 1 wt.%, but is preferably at least 0.1 wt.% of the total fat content. Since it was found that AA is particularly effective in reducing intestinal permeability impermeable walls, the composition according to the present invention includes a relatively high amount, preferably at least 0.1 wt.%, even more preferably, at least 0.25 wt.%, most preferably, at least 0.5 wt.% of the total fat content.

Preferably the composition according to the present invention includes from 5 to wt.% polyunsaturated fatty acids of the total fat content, preferably from 10 to 50 wt.%. LC PUFA with 20 and 22 carbon atoms can be provided as free fatty acids in the form of triglycerides in the form of a phospholipid, and as a mixture of one or more of the above. The composition according to the present invention includes at least one of AA and DHA in the form of a phospholipid.

To ensure adequate nutrition for the child, the composition of the present invention preferably includes a lipid, protein and digestible carbohydrates and preferably is introduced in liquid form. Used in the present invention, the term "liquid food product includes a dry food product (e.g., powders), which is accompanied by instructions for mixing the above-mentioned mixture dry Peshev is the first product with a suitable fluid (e.g., water).

It was found that stool can be further reduced by the introduction of the composition according to the invention in the form of a liquid food product with an osmolarity of from 200 to 400 mosmol/l, preferably from 250 to 360 mosmol/l, preferably from 250 to 300 mosmol/l, where the composition contains the intact proteins (for example, non-hydrolyzed). Preferably the liquid food product has a caloric content of from 0.65 to 0.9 kcal/L.

Examples

Example 1: reduction of pH in faeces of children

Children n who were given baby food, containing no additives (control); galactooligosaccharide (GOS) (Vivinal-GOSTM, Friesland Foods, The Netherlands the combination of GOS (Vivinal-GOSTMand inulin (RaftilineTM, Orafti) (GOS/inulin) and breast milk (BM). After 16 weeks was determined by the pH of faeces (see Table 1).

The above data indicate the effect of reducing the pH of GAL-oligo according to the invention and demonstrate the advantage of using GAL-oligo according to the invention in a mixture of baby food with low content of threonine, palmitic acid. Effective reduction of pH demonstrated by using a combination TOS and inulin, which indicates the effectiveness of two different oligosaccharides in the composition of the invention.

Example 2: Production of lactate from GAL-oligo

Investigated the ability of inulin, galactooligosaccharides (GOS), breast milk oligosaccharides (HMO) and a combination of GOS and inulin to stimulate the production of lactatein vitroin p loginmessage periodic fermentation system using the faeces of infants.

Fresh faeces were obtained from healthy children who are bottle-feeding, and suspended in 500 mg of various prebiotics (see Table 2). As the source of the GOS used Vivinal GOS (Friesland Foods, The Netherlands); as a source of inulin used RaftilinHP (Orafti); and used a dedicated breast milk oligosaccharides (HMO). From the results it is clear that the introduction of GAL-oligo according to the present invention results in the production of large quantities of lactate. This indicates the advantage of using GAL - oligo in the composition and method according to the invention.
