Prediction of threatened reduced erythrocyte oxygenation in pregnant woman with bronchial asthma

FIELD: medicine.

SUBSTANCE: threatened reduced erythrocyte oxygenation in a pregnant woman suffering an acute attack of bronchial asthma in the first trimester of gestation is predicted by evaluating oxyhemoglobin (HbO2) and 2,3 DPG (2,3 diphosphoglycerate phosphatase). This is followed by calculating a D discriminator value by formula D=18.05·2,3DPG + (-0.075·HbO2), and observing the D value within 88.66 to 102.42, threatened reduced erythrocyte oxygenation that leads to hypoxia is predicted.

EFFECT: use of the method enables predicting threatened reduced erythrocyte oxygenation in a pregnant woman with the acute attack of bronchial asthma.

The aim was to study the conditions of the threat of oxidation of hemoglobin in erythrocytes in peripheral blood of pregnant women in the first trimester during exacerbation of bronchial asthma.

In bronchial asthma, a marked disturbance of gas exchange (Avenatti, 2007, Enteredinto, 2005).

One of the indicators of the development of hypoxia in pregnancy is increased in peripheral blood erythrocytes of 2.3 FGD (Aripov A.A., 1977; Veretennikov E.N., 2004). However, in the available literature not covered the question of the formation of hypoxia in pregnant women with acute bronchial asthma and no information covering how reduced the percentage bO2pregnant women with asthma and what role is played by the change in the erythrocyte content of 2.3 FGD.

The claimed method has the following methods.

1. Investigated peripheral blood of 20 pregnant women in the first trimester of gestation who were admitted in the period of exacerbation of bronchial asthma. Research was conducted on the basis of hospital obstetric clinic of the far Eastern scientific center of physiology and pathology of respiration SB RAMS. All studies were conducted with regard to the requirements of the Helsinki Declaration of the world Association "Ethical principles for medical research involving human subjects"with amendments 2000 and the good clinical practice in the Russian Federation", approved by Order of Ministry of health of the Russian Federation dated 19.06.2003, No. 266.

2. Content bO2in peripheral blood erythrocytes of pregnant women was determined on spectrophotometer "Stat-Fax - 1900+" (USA) Moscosco (1968).

3. The content of 2,3 FGD (2.3 diphosphoglycerate-phosphatase) in peripheral blood erythrocytes of pregnant women was determined on spectrophotometer "Stat-Fax - 1900+" (USA) by the method of Ellinogermaniki, Shubhranshu and Gvers (1980).

4. The content of the herpes virus was determined immunofermentnym method on spectrophotometer " Stat Fax-2100" (USA).

5. The content of 2,3 FGD and bO2during exacerbation of bronchial asthma in the red blood cells of pregnant women were processed by the statistical method of discriminant analysis.

Studies have shown that the exacerbation of asthma in pregnant women in the first trimester of gestation with increasing content of 2.3 FGD to 6.05±0.25 mmol/l (control - 4,46±0.32 mmol/l) reduced the saturation of hemoglobin with oxygen: bO2reduced to 90,40±2,60% (control - 95,90±2,60%).

During exacerbation of asthma during the first trimester of gestation according to calculations discriminant formula D=18,05·2,3 FGD+ (-0,075·bO2) when the value of D from 88,66 to 102,42 projected threat reduction hemoglobin saturation of peripheral blood erythrocytes of pregnant oxygen that drive the t to the formation of hypoxia.

EXAMPLE

Pregnant N at week 12 of gestation with acute exacerbation of bronchial asthma. Within 10 days, she daily took the blood, which simultaneously determined the content of the b2and 2.3 FGD. It is noted that the average content b2a pregnant woman fell to 93,52%and 2,3 FGD increased to 6.7 mmol/l On the basis of the obtained data constructed discriminant equation D=17,0·2,3 FGD+(-0,080·HbO2), where D=98,75. Because of this a pregnant woman predicted the threat of reducing the saturation of red blood cells with oxygen.

The method of new diagnostic criteria is simple, work time expenditures to evaluate threshold of table salt gustation are 10 minutes. Haptoglobin genotype is identified with the home equipment with work time expenditures taking about 2 hour. Identification of haptoglobin genotype and threshold of table salt gustation (TTSG) precede pharmacotherapy. If prior to the treatment, haptoglobin genotype either 1-1, or 1-2 is observed with medium threshold of table salt gustation <0.32%, prognosis for hypotensive therapy is considered to be favourable. And if prior to the treatment, haptoglobin genotype 2-2 is identified, and threshold of table salt gustation is high >0.32%, prognosis for hypotensive therapy is unfavourable.

Hemoglobin level is multiplied by 0.809, erythrocytes deformability index is multiplied by 3.247 and the two products are summed. Vegetative Kerdo index value is multiplied by 0.804, non-enzymatic fibrinolysis duration is multiplied by 0.365 and the products are summed. Method is based on determination of general oxidant activity (GOA) and general antioxidant activity (GAA) with further determination of oxidant index (OI), which equals ratio of GOA to GAA. In accordance with the invention oxidant activity is determined by degree of echinochrome A oxidation by components of oxidant system of blood serum or plasma, and general antioxidant activity is determined by degree of echinochrome A oxidation with chloramine B, added to blood serum or plasma. OI index of healthy donor is taken as a unit. OI value higher than a unit testifies to disbalance of general oxidant status of organism. Method ensures up to 20 fold reduction of amount of analysed serum or plasma in comparison with standard methodology, reduction for carrying out analysis from 72 hours to 2 hours. When ranging it is possible to perform analysis of more than 500 samples during one day, using microboards and microplate spectrophotometers.

EFFECT: method is simple in implementation, economical and does not require large volume of thermostatically controlled chambers.

SUBSTANCE: patient's blood is analysed for the erythrocyte membrane glycophorin concentration by gel electrophoresis assay and the malonic aldehyde concentration by spectrophotometric analysis. Then a discriminant equation D =-2.719 · glycophorin +{+25.331 · malonic aldehyde} is solved. If D is equal to or exceeds a boundary value +9.52, higher glycophorin activity in erythrocyte membranes is predicted in pregnant women.

SUBSTANCE: for diagnostics of early manifest congenital syphilis values of cellular, humoral link of immunity and phagocytosis in acute period of disease are determined. If values of relative number of CD8+ - subpopulation of T-lymphocytes are over 20.0% and phagocytic number of neutrophils is over 4.2 units with respect to control group, and if values of IRI (CD4+/CD8+) is lower than 2.9 and relative content of CD95+ - lymphocytes is lower than 19.6%, than in control group, early manifest congenital syphilis is diagnosed.

SUBSTANCE: invention relates to laboratory methods of investigations. Method involves sampling specimen from patient to be inspected, extraction of serotonin and histamine from a specimen, chromatography of extract and determination of concentration of serotonin and histamine by the fluorescence intensity value. Saliva is used as biological fluid. Saliva by volume 1 ml is extracted with 4 ml of 1 N hydrochloric acid solution, 2 g of anhydrous potassium carbonate and 5 ml of mixture of butanol and chloroform in the ratio 3:2 are added, extract is shaken up and centrifuged. Organic phase (4 ml) is sucked off from extract and passed through chromatography column (diameter is 3 mm, height is 16 mm) filled with ion-exchange resin KB-4 or KB-4P-2 or Bio Rex-70 in H+-form, size of granules is 0.1 ± 0.02 mm. Histamine is eluted with 4 ml of 0.1 N hydrochloric acid at the rate of eluting solution 0.4 ml/min. Histamine concentration is determined by reaction with ortho-phthalic aldehyde dissolved in ethanol. Serotonin concentration is determined by reaction with ninhydrin in organic passed through column. Method provides assaying the saliva concentration of serotonin and histamine with high precision.
