Purulent wound healing technique

FIELD: medicine.

SUBSTANCE: invention refers to medicine, particularly to conservative purulent wounds healing. A technique implies that the wound is locally managed with electric pulse activated water containing metal oxide nanostructure dispersions. Water is prepared with using copper and silver electrodes, at copper density 30 % and silver density 70 %, current action 50 Hz and total discharge energy 10 J/l. The wound is managed and covered with water 1-2 times a day.

EFFECT: technique results in rapid relief of the pyoinflammatory process in the wound due to antimicrobial and regenerating effects, is not accompanied with by-effects.

 

The invention relates to medicine, namely to the conservative treatment of purulent wounds.

Despite the rapid development of modern medicine and the success of pharmacology, the problem of treatment of purulent wounds never ceases to be relevant, as the issues of effective influence on the course of the wound process are solved not always successfully.

Currently used for local treatment of purulent wounds many modern antiseptic and antibacterial agents are often ineffective or inefficient, often have side effects that cause immunosuppression.

Analogue of the invention is the use of antiseptics, such as chlorhexidine digluconate of 0.05%.

The objective of the invention to develop a method for effective treatment of purulent wounds in the 1st and 2nd phases of the wound healing process.

The method is as follows: after debridement performed as reorganization of the hearth and the floor of the wound surface with water containing a dispersion of an oxide of metal nanostructures obtained by electro-activation using copper-silver electrodes (the specific content of copper - 30%, silver 70%), electric current of 50 Hz and a total energy of digits - 10 j/L.

Similar means in modern medical practice is not used, the active component presents aqueous dispersions of oxide nanostructures of metals, obtained by electro-activation using copper-silver electrodes (the specific content of copper - 30%, silver 70%), electric current of 50 Hz and a total energy of discharges 10 j/L.

We first identified the antibacterial effect of the solution by use in the treatment of experimental wounds. The experiments conducted on 150 rat Wistar weighing 110-130, Made 4 series chronic experiments. All series of chronic experiments assessed the wounds on clinical criteria daily from the 1st to the 14th day of the treatment (time cleansing wounds, the appearance of granulations, deadlines wounds with granulation, the beginning of epithelialization), investigated laboratory parameters blood was determined by quantitative composition of microflora was assessed by qualitative composition of microflora in the wound, was determined by the local immune status, level wound healing ability of the study and control drugs, fence wound biopsies at 3, 5, 7, 10 and 14 days, and statistical processing of the obtained data. Studies have shown that in 3 and 4 the experimental series, where he used water containing a dispersion of oxide is s nanostructures of metals, the decrease of leukocyte count and ESR are marked with 3-5 days, and in the control series, where he used a solution of chlorhexidine digluconate of 0.05%, a pronounced leukocytosis was observed up to 6-8 days, ESR up to 10 days.

The overall level of wound healing capacity in % for the control series in the treatment of 0.05% chlorhexidine digluconate - 92,53, in series in the treatment of water containing a dispersion of oxide nanostructures of metals - 94,79. The greatest reduction of the area of the RAS, 3 and 4 experienced a series noted in the 1st 5 days, i.e. in the first phase of wound healing.

In the treatment of electro-activated water containing a dispersion of an oxide of metal nanostructures, with a 3 day growth of microorganisms was absent. Microbial colonization of 4 experimental series, in the treatment of water containing a dispersion of an oxide of metal nanostructures, in the course of treatment was progressively decreased, which was not observed in the control series.

Histological study of wound biopsies at 3, 5, 7, 10 and 14 days, it was noted that best results are obtained when using water containing a dispersion of an oxide of metal nanostructures.

Thus, studies have shown the possibility of using water containing a dispersion of an oxide of metal nanostructures obtained by electro-activation is used is to eat the copper-silver electrodes, in the treatment of purulent wounds in both the first and second stages of wound healing, proved its regenerative and anti-microbial effects.

Antibacterial effect of electro-activated water containing a dispersion of oxide nanostructures of metals, expressed as the ratio of aerobic and anaerobic bacteria. When applied topically to treat wounds in the first phase of wound healing drug has antibacterial, General and local immunostimulatory effect in the second phase of wound healing normalizes microcirculation, promotes regeneration, maturation of granulation tissue and the development of epithelialization.

Local application of electro-activated water containing a dispersion of oxide nanostructures of metals with antimicrobial activity, avoids systemic side effects, reduce the risk of development of allergic diseases and to achieve the maximum concentration of the drug directly into the wound.

Using this method provides the following advantages: can be used as a local treatment of purulent-inflammatory wounds of soft tissues, as the impact provides an antibacterial effect, can reduce the intensity of symptoms of purulent-inflammatory processes in the wound without the addition of antibiotic therapy and reduced the duration of treatment of purulent wounds. Thus, the use of water containing a dispersion of an oxide of metal nanostructures, leading to rapid relief of the inflammatory process in the first phase of wound healing, accelerating the regeneration of wounds in the second phase and is not accompanied by side effects.

The method of treatment of purulent wounds, including local use of electro-activated water containing a dispersion of an oxide of metal nanostructures obtained using copper-silver electrodes, when the specific content of copper 30% silver and 70%, the influence of an electric current of 50 Hz, and the total energy of discharges 10 j/l, the treatment of the wound with water and apply to the wound surface water is performed 1-2 times per day.

SUBSTANCE: invention relates to an aqueous polyurethane-polycarbamide dispersion (PUR-dispersion), used for coating substrates, as well as for preparing mixtures and adhesives. The dispersion has two separate maxima in particle size distribution. The maximum ratio of fine fractions lies between 51 and 150 nm and the maximum ratio of coarse fractions lies between 160 and 700 nm. Said PUR-dispersion contains external emulsifying agents, has low viscosity, is stable during storage and transportation and solidifies at relatively low temperatures between 120C and 150C in 2-3 minutes.

EFFECT: coatings obtained from such PUR-dispersion have high adhesion strength in wet state, high wear resistance, scratch resistance, flexing resistance and hydrolysis resistance.

SUBSTANCE: invention relates to medicine, namely, to surgery, and may be used for treatment of patients with localised pyoinflammatory diseases of abdominal cavity or retroperitoneal space. For this purpose, transcutaneous drainage of cavity of pyoinflammatory focus is carried out with subsequent evacuation of its content. Then fractional lavage of cavity is carried out, and solution of antiseptic is introduced in it in volume of up to 80% of volume of evacuated exudate. Drainage is closed and left as such for 12 hours. Afterwards drainage is opened, fractional lavage is carried out, and a solution of antiseptic is repeatedly introduced into cavity with subsequent closure of drainage for 12 hours up to the following sanitisation. Manipulations are repeated until complete reduction of ulcer.

That is ensured by the surgical intervention and starting from the early postoperative period, bleach solution is administered intravenously and locally. The intravenous introduction of 0.004% bleach solution is carried out once a day at 10 ml of the solution per 1 kg of body weight. Additionally, 0.06% bleach solution is introduced locally intrarenally with a nephrostomic drainage every 12 o'clock in amount 50 ml in divided portions no more than 4 ml for one portion. The therapeutic course is 7 days.

EFFECT: method provides reduced duration of staying in hospital, reduced postoperative complications of septic nature and reduced duration of healing of postoperative wounds in the patients.

For this purpose subcutaneously introduced is sandostatin in dose 100 mkg 5 times per day and roncoleukin in dose 500 thousand IU 1 time per day. Introduction is realised for 2 days and 3 days after operation, as well as in 1 and 2 weeks of post operational period. Simultaneously on the same days during 1 hour hyperbaric oxygenation is carried out with partial pressure of oxygen 1.5-2 atmospheres.

EFFECT: method allows to reduce frequency of fatal outcomes in patients with said pathology due to immunomodelling action, which appears as a result of definite combination of pharmacological medications and hyperbaric oxygenation.
