The method relates to medicine and can be used for the correction of overweight and in the treatment of obesity.

Overweight and obesity are characterized by an imbalance between intake and expenditure of energy substrates. One of the reasons leading to the accumulation of excess fat is to reduce consumption and the body in the background of normal or excessive food intake. The main principle of treatment of obesity lies in shifting the balance of energy consumption - energy consumption", as in the reduction of energy consumption and increase energy consumption. On this basis, in the treatment of obesity using the following approaches: fasting, diet therapy, exercise (physical therapy), drug therapy, physiotherapy, psychotherapy, surgical methods of treatment (1, 2).

Despite the increasing number of methods and tools aimed at the correction of body weight, their effectiveness remains quite low. According to the literature (3) only 5-10% of patients retain achieved during treatment body weight within 2 years. One of the reasons of low efficiency of obesity treatment is to reduce the level of metabolic processes in response to long-term dietary restriction and, as a consequence, the slowdown in reduction of mass of the body and re-set weight.

There is a method of treatment of obese patients with neuroendocrine form hypothalamic syndrome (4). Patient conduct intermittent fasting, including 3-5 days of famine and 3 days recovery period. After a course of fasting patient is injected with 0.02%sodium hypochlorite solution intravenous drip for 200-400 ml 1-2 days 3 times. The proposed method of treatment allows to obtain a stable effect of weight loss with normalization of lipid and carbohydrate metabolism. The disadvantage of this method is that its application is limited to a patient's neuroendocrine form hypothalamic syndrome, which accounts for only 5-10% of people with obesity. In addition, for the production of sodium hypochlorite required apparatus electrochemical detoxification of the body EDO-3. Another disadvantage of this method is the possibility of its use only in hospital.

There is a method of reducing body mass in patients with hypertension 1-2A stage in combination with alimentary-constitutional obesity (5). Patients eliminate the use of water and water treatment in the first 24-48 h of starvation (absolute hunger). In the next 10-12 days patients are under the usual scheme of medical starvation, but limiting daily water intake to 10-12 ml/kg of This method allows to increase the reduction of body weight with 0,and-0.6 to 0.8-1.2 kg/day, to hasten the onset of acidotic crisis and early active mobilization of fat from the depot (2-3 days), to achieve earlier timing stabilization of physiological parameters of blood pressure (2-5 day). As a result, the period of inpatient treatment is reduced in 1,5-2 times. The disadvantages of this method include the fact that prolonged fasting 12-14 hours in addition to the loss of adipose tissue leads to severe loss and muscle tissue, which is the main body, utilizing both Exo-and endogenous fat. In addition, the disadvantage of this method is that the reduced level of energy exchange in prolonged starvation leads to the decrease in the rate of weight loss. In the phase of the output of fasting without proper diet usually develops a rebound effect, i.e. re-set weight, sometimes higher than before treatment. Another disadvantage of this method is that long-term absolute hunger is contraindicated in a number of diseases and can lead to exacerbation (gastro-intestinal tract, such as gastritis, gastric ulcer disease; gout, heart rhythm disorders, sleep disorders).

There is a method of treatment of alimentary obesity, including psychotherapy in the waking state of the patient, while using ve the ball suggestions reinforce and strengthen the motivation to reduce weight, then enter the patient into a state of meditation, exercise autogenerative impact, inspire indifference to food, cause anorexics-dispatches feeling and enhance their mechanical irritation of the anterior projection of the stomach, esophagus. Session repeated after 1-2 days. The last session started with irritation using acupuncture or acupressure reflex zones or points of the hypothalamic-hypofyse system, autonomic nervous system and gastrointestinal tract. In the program of introducing into a meditative state include weight reduction, which is pre-calculated with the parameters of weight reduction for the first two sessions and repeat autogenerative impact on the background of irritation reflex zones (6). The method requires at least three sessions of psychotherapy. The known method has the same drawbacks as the previous, in part, on the restriction of the power and response of "rebound" after the end of treatment. In addition, the mechanical irritation of the projection area of the stomach and esophagus leads to undesirable consequences in individuals with diseases of the gastrointestinal tract, in particular with gastro-esophageal reflux, peptic ulcer and other

There is a method of treatment of obesity, including indirect is sychotherapy in an atmosphere of light support during which the patient talk about the impact halocline therapy on the human body, in the process of food offerings include table lamps on both sides of the Cutlery on one side green on the other side is red so that the light from them was sent on a plate with a high-calorie product that causes the patient has an excessive appetite. Conduct behavioral psychotherapy, aimed at shaping aversion to the specified product by rendering this product and causing repulsion of objects. Then conduct group training psychological stability 10 times in 2 hours a day (7). The disadvantages of this method include the fact that one of the patients is resistant to psychotherapeutic effect, the other is overly sensitive, which leads to permanent aversion of food and subsequent exhaustion. In addition, aversion to certain foods cause patients psychological discomfort both during and after treatment.

There is a method of treatment of obesity, including diet therapy and psychotherapeutic effect when carrying out collectively-group sessions, with the main psychotherapeutic effect is carried out in two sessions of eight hours each method marathon in conditions which asthenia, achieving failure biorhythms and behavioral stereotypes, thus provide motivation for healthy eating, self-confidence and faith in success, form a weight loss program, visualize and concretize treatment, keep a diary of eating behavior, exercise psychological protection in the event of violations of diet; after the main sessions performed psychotherapeutic effects in a single session lasting three hours after one and three months, aimed to consolidate the obtained results, discussion and exception situations that provoke a relapse of obesity (8). The disadvantages of this method include the fact that asthenia and failure of biorhythms in the following may cause an exacerbation of chronic diseases, exacerbating limnologically disorders, which are quite common in individuals with obesity.

There is a method of treatment of obesity, including preliminary daily fasting, psychotherapeutic effect in the waking state by the method of group rational psychotherapy and diet therapy with the exception of energy-dense foods, while on stage a group of rational therapy associate the effect with individual optimization of weight due to preregulation self-regulating systems of food centre, after which the state of the honey the state provide direct and indirect suggestion influence and form the active positive emotional-volitional motivation perform dietprogram, reconstructing and subjecting the motivational scheme of social motivations, which are used psychosomatic paradigm in the form of short verbal fabul and simultaneously reinforce their reflex effects in the forehead and temporal lobes with fixation of physical pressure in epigastrium, with the amount of food to limit 1300-1500 ml per day and eliminate high-calorie foods in 2-3 months (9). This method is not effective enough, because it seeks only to limit receipt of food ingredients without increasing the body's energy. In addition, long-term dietary restrictions lead to lower level of metabolic processes (including splitting deposited in body fat), which helps in further weight gain. In addition, as mentioned above, some patients are resistant to psychotherapeutic effects.

There is a method of treating obesity, comprising a preliminary discussion of therapist with the patient about the nature of the disease, therapy, loss program weight, while doing the installation of the patient to an active position for treatment, for each positive statement sets out the "kinesthetic anchor by touching the "leading" the patient's arm(right - to - the right-handers, left - handers). Then a treatment duration of 12 weeks, consisting of a combination of fasting and nerazrushaushsego power modes, which are alternated so that one week was five discharge and two nerasprodannyh day, food in fasting days is carried out using nutrient mixture Metabolin instead of Breakfast, lunch, dinner and over night; in the intervals between the reception of the mixture permits the use of vegetables in raw or boiled (except potatoes) and fruits (except bananas and grapes); nerasprodannye days of food is carried out in a normal mode without increasing the flour and sugary foods and to reduce daily fat intake to 35-40 g, in this first course of treatment within 14 days, and then in nerasprodannye days appoint a mixture of: figs, apricots, prunes in a ratio of 1:1:1 two tablespoons in two hours after dinner. After 12 weeks and up to 6 months conduct following courses with the purpose of a low-calorie diet low-calorie 1200 kcal in the first month, with a further increase to a value corresponding to the normal needs of the body, during the 6 months of treatment psychotherapist and nutritionist conduct dynamic monitoring of the patient to monitor weight loss, diet using the established "kinesthetic anchor is" to consolidate the positive results and maintain complex recommended therapy (10). The known method is not effective enough, as directed only to limit receipt of food ingredients without increasing the body's energy. In addition, some patients is resistant to psychotherapeutic effects, and the use of low-calorie diet, as mentioned above, lead to a lower level of metabolic processes, which helps in further weight gain. For this reason, the method involves prolonged observation of the patient of the psychotherapist and the need for additional psychotherapeutic influence.

Closest to the claimed method is Tamaz Mchedlidze correction of the weight of the person, including the diary of eating behavior, split meals 4-6 times per day and limit consumption of salt, sugar and flour products; performing dosed physical activity, conducting heat therapy and massage, based on the techniques of classical, Thai, massage and neurosedative. At the first meal of the patients consume high-calorie carbohydrates with a high glycemic index, second, third and fourth techniques - low-calorie carbohydrates with low glycemic index; the second half of the day move on protein foods. Physical activity is first performed at a moderate intensity when the size of the pool is CA 50-60% of maximum heart rate, and then with the average intensity when the value of the heart rate of 60-70% of maximum heart rate. The value of the maximum heart rate calculated by the formula: 208-(0,7×X), where X is age. Heat treatments performed before going to sleep at a temperature of 70-80°C, followed by dipping in cold water. Physical activity of moderate intensity exercise walking, swimming, Cycling and walking and when doing light aerobic exercise and physical activity with moderate intensity exercise during race walking, vigorous swimming, with the active occupation of fitness. Physical activity are at least three lessons per week of 45 minutes Massage includes at least one of the following methods: lymph drainage techniques, kneading, neurologie manipulation, stroking. Thai massage includes at least one of the following methods: acupuncture bioactive points padded thumbs, pressure hands, feet. Moderate food restriction effect of weight loss reaches combination of diet and exercise, thermomechanical and massage that increases the level of metabolic processes (including splitting deposited in body fat) (11). The disadvantage of this method is that the patient has contraindications to the use of thermotherapy and massage (ischemic heart disease, arter the social hypertension, diseases of the joints, veins, benign growths, such as fibroids, and other). Another shortcoming is the unavailability of massage, Thai and neurosedative massages for a wide range of patients because of its high cost or lack of such services by place of residence (in the clinic or specialized center). In addition, the termination of the execution of physical exercises combined with massage and termolecular often leads to the return of the weight.

The problem to which the invention is directed, is to increase the availability of way for patients, reducing contraindications, achieving a consistent effect of weight loss.

The solution of this problem is achieved by the fact that patients are prescribed 3-4-day fasting without limiting fluid intake, then taking low-fat or fat-free products within one to two days of hunger, then the daily consumption of low-calorie food 1000-1200 kcal/day up to 12-14 days from the start of treatment; daily exposure to the patient by a person full spectrum artificial light with a light intensity of at least 2000 Lux or natural within 1-2 hours a day simultaneously or alternately with daily dosed physical load.

Description of the invention

The way VC is uchet 3-4-day fasting patient without limiting fluid intake, then the reception low-fat or fat-free products within 1-2 days of hunger. In the days that followed daily until 12-14 days inclusive from the beginning of the treatment to consume low calorie food 1000-1200 kcal/day. From the first day of exercise effects on the patient side face full spectrum light artificial light intensity of at least 2000 Lux for 1-2 hours a day simultaneously or alternately with daily dosed physical load.

Dosed physical load is prescribed to increase heart rate not higher than the threshold values defined by the known methods, for example, according to (11) or table 1, after examination of the patient, technician, ECG recording and in the absence of contraindications.

Long periods of fasting and exit hunger determined depending on comorbidities. When fasting in the first 3-4 days for drinking use, for example, water, broth hips, decoction of oats, etc. At the exit from starvation in the next 1-2 days patients as low-fat or fat-free products recommend dairy products (e.g. yogurt low fat), salad, mucous broths, such as rice, etc. as a low-calorie products to 12-14 days inclusive can be used with the products shown in the attached version of the basic neither is kcalories diet or similar calorie content.

An example of a basic low calorie diet for obese patients (1000-1200 kcal / day)

Breakfast 1: kasha (buckwheat, oats, barley), rye bread, tea, coffee, juices vegetable or fruit.

Breakfast 2: the carrot or an Apple or a pear.

Lunch: soup vegetarian (vegetable broth); raw or steamed vegetables (cabbage, carrots, tomatoes, cucumbers, onions, green onions, parsley, lettuce, served with vegetable oil; boiled meat (or chicken, steamed); compote of dried fruits (without sugar).

Afternoon tea: tea spoon fruit-nut mixtures (e.g., prunes, dried apricots, raisins, walnuts, lemon, honey).

Dinner: fish, lean meat (boiled or steam) or soy products, vegetable stew or side dish of cereals (buckwheat, oats, barley), tea green.

When exposed to artificial light use full spectrum lamps (fluorescent lamps) light intensity of at least 2000 Lux or natural light outdoors. The exposure duration is 1-2 hours a day. The distance from the patient's face to the light source 50-60 cm, the patient should be opposite the light source, it is possible not to look at him.

When using natural light, the patient should receive daily 1-2 hour walks outdoors depending on light intensity (e.g., one hour PR is Sunny or 2 hours - in cloudy weather). The intensity of light in cloudy weather, as a rule, is 2000-2500 Lux, sun - up to 10,000 Lux.

Exposure to natural or artificial light is desirable in the morning or the afternoon, so as not to cause failure of circadian rhythms of the body of the patient. The duration of the course either 12-14 days.

As physical activity use walking, race walking, etc. or active movement on a small vehicle, such as Cycling, water Cycling, sports wheelchair without automatic turn driven independent effort of the patient, etc.

The venue for physical activity can be used by the simulator, for example, in the form of a treadmill, and others In the early days of treatment (hunger) exercise can be easier with the increase to moderate in the coming days. An example of a mode selection physical activity throughout the course correction weight depending on the age of the patient are presented in table 2. During exercise you must make sure that the heart rate was in the range corresponding to the age and size of load. In the presence of severe shortness of breath and excessive sweating, the patient is recommended to reduce the load or to stop and rest.

As an additional, but not required is compulsory for achieving the stated effect can be used in the absence of contraindications water treatments (for example, power shower), physiotherapy (electrical myostimulation, massage). Contraindications to these impacts are allergic skin diseases, dermatitis of a different nature, varicose veins, benign tumors (fibroids, breast), progressive hypertension, presence of a pacemaker, etc. is Possible, but not necessarily the application of methods of rational psychotherapy, aimed at promoting a healthy lifestyle.

After the end of treatment, patients are advised to follow a low calorie diet of 1200-1500 kcal/day, and daily walking in the light of day for at least 40 min a day.

Examples of specific performance

1. The individual examples of the method

Example 1.

Patient K., aged 45, body weight at admission 101 kg, height 164 see Diagnosis - alimentary-constitutional obesity of 2 degrees. Comorbidities: neurocirculatory dystonia of hypertensive type, chronic cholecystitis. Carried out the correction of body weight according to the claimed method in the hospital, including a 14-day course of fasting-diet therapy (3 days of starvation, 1 day from hunger, with the further transition to a low-calorie food within 1000-1200 kcal/day), phototherapy using fluorescent lamps, the intensity of 2000-2500 Lux, the impact of the journey at a distance of 50-60 cm for 2 hours a day. In addition, the treatment on the entire course was used daily exercise - walking on the treadmill trainer or sports group physical therapy - the total duration of 1-2 hours a day. For course correction, body weight decreased by 4.5 kg. body Weight after treatment 96,5 kg

After treatment at the outpatient period, it was recommended to follow a low calorie diet within 1200-1500 kcal/day, daily walks in the daytime duration of not less than 40 minutes by Following these recommendations, in the past after the end of treatment 3 months, the patient had reduced body weight even at 2.7 kg

Example 2. Patient R., aged 35, weight 75 kg, height 170 cm, the diagnosis of overweight. Concomitant diagnosis: chronic bronchitis without exacerbation, hay fever. Asked about the seasonal increase in body weight. The patient notes that over the past 3 years in the autumn-winter period, the body weight increases by 3-5 kg for 12 days correction of body weight according to the claimed method. Was assigned to dietary treatment, including 4 days of hunger, 2 days of withdrawal from hunger, the subsequent consumption of low-calorie products 1000-1200 kcal/day. The patient was prescribed physical activity simultaneously with exposure to natural light in the form of daily walks in SV is tlae time of day (preferably in the first half of the day) for 1-2 hours a day. During the course of treatment the body weight decreased by 1.5 kg

Further recommendations included the use of low-calorie food 1200-1500 kcal/day, walk in the daylight lasting at least 40 minutes each day. 3 months after the end of treatment and compliance with these recommendations, the body weight of the patient decreased by 4.5 kg and was 69.5 kg, which corresponds to normal.

2. Group research

Corrected mass body according to the claimed method in comparison with other widely used methods for correction of body weight. This was allocated to 3 groups of women with obesity. Group I - 26 patients with a body mass index (BMI) and 36.2±1.63 kg/m2group II - 30 women with a BMI 35,9±1.63 kg/m2the group III - 24 women with a BMI of 37.7±2,23 kg/m2.

The common pattern of treatment was: 4 day fasting without limiting fluid intake, including broth hips, with subsequent 2-day withdrawal from hunger (600 ml kefir daily) and a further transition to a low-calorie (1000-1200 kcal/day) diet. All patients received the same water (shower) and physiotherapy (electrical myostimulation). In group I, other appointments were not. In group II was additionally used daily aerobic exercise (40 minute walk on the treadmill and 30 minutes in gr is the group of physical therapy).

In the group III held the correction of body weight according to the claimed method. Patients were assigned to 4-day fasting without limiting fluid intake, including broth hips, with subsequent 2-day withdrawal from hunger (600 ml kefir daily) and a further transition to a low-calorie (1000-1200 kcal/day) diet. While walking on a treadmill at the same time held sessions of phototherapy according to the claimed method: patients affected by light full spectrum fluorescent light with a light intensity of 2,000-2,500 Lux at a distance of 50-60 cm from persons during the duration of the exposure to 40 minutes

The effect of light therapy on the activity of metabolic processes (metabolism) was evaluated by changing the number of the consumed oxygen as the primary oxidant metabolic processes. To determine the power consumption in the quiescent state used indirect calorimetry - oxysporium, in terms of thermal comfort (temperature +20°C) and relative peace, 1.5 hours after Breakfast and after 20 min of rest. In accordance with the methodology proposed Kataev NN. (12), the subject in the sitting position for 1 min adapted to breathing through the mouthpiece, and the front of the faucet, then the last level quiet exhalation connected with the respiratory circuit of a spirograph. After the instrument is placed surveyed the spirograph after 5-7 min of quiet breathing was determined by oxygen consumption.

In the two weeks of treatment in group I, the body weight decreased by an average of 3.0±1,1 kg At this level2tended to decrease and amounted to 248,5±34,3 ml/min before treatment and 233,4±29,2 ml/min (p=0,14) by the end of two weeks of treatment. This group demonstrated the process of reducing the level of energy exchange in response to calorie restriction diet. In group II body weight decreased or 4.31±1,12 kg, the level of oxygen consumption tended to increase from 242,1±24,2 up 256,1±42,4 (p=0,22). In group III, the weight loss was most pronounced 4,93±1.02 kg, while increasing the level of oxygen consumption with 252,8±31.9 to 303,2±68,4 ml/min (p<0,001) or 120% relative to the initial level.

The duration of the activating effects of light therapy on metabolism was identified in one case. So the level2the original was 212,3 ml, immediately after the session, he increased to 279,1 ml (to 131.4% to the original), after 1 hour after light exposure he was 266,9 ml (125,7% to the original), after 2 hours of 254,8 ml (120% of the original). It is seen that the impact of the light is sufficiently expressed tracking the reaction to the energy exchange. The combination of physical activity with phototherapy on the background of a low-calorie diet allows you to achieve a sustained reduction in body weight of the patient.

Advantages of the claimed method are that it can be used in the IAOD in patients with concomitant diseases, in which combination of physiotherapy, massage, intense physical exercise. The method provides a stable effect of reducing body weight, while psychological correction is not required, which is important for patients who refuse psychological impact or resistant to it. The method is easily reproducible, can be used in individuals with disabilities to perform physical activities, including individuals with limited mobility, such as wheelchair users.

SUBSTANCE: invention refers to medicine and hygiene. A leg support comprises a base, a bearing part, sound and light indicator elements or a computer interface, a control member and sensors. The bearing part enables the ankles to be flexed and extended with controlling the feet movement amplitude meanwhile by the setting height of the bearing part relative to the support base. The sensors are used to determine greater amplitude feet flexion and extension events and allow to monitor frequency and number of such movements. The control member is capable to compare the frequency and number of feet movements during certain time intervals. The frequency and number of feet movements are compared to the preset parametres that enable to prevent blood congestion and clotting in feet. If observing the divergence of the parametres from the preset values, the control member is capable to activate the sound and light indication, or to display the information on the monitor to remind a user of the necessity of resumption of the preventive exercises.

EFFECT: essential simplification and cost reduction of the device.
