Method of treating burns of conjunctiva

FIELD: medicine.

SUBSTANCE: in early after-burn period, with underlying antiinflammatory, antibacterial and trophic treatment, additionally alprostadil in single dosing 40 mcg dissolved in 200 mg of physiologic saline is introduced intravenously drop-by-drop within 2 hours.

EFFECT: method allows for conventional and effective treatment of burns of the conjunctiva ensured by effect of alprostadil on pathogenic links of the burn disease.

The invention relates to medicine, namely to ophthalmology.

There is a method of treatment of eye burns (Puchkovska N.A. and other Pathogenesis and treatment of eye burns, Moscow, Medicine, 1973, p.75-101).

To date the medication of prostaglandin E1 (alprostadil) in the treatment of eye burns was not applied. The most widely used drugs of prostaglandin E1 (alprostadil) obtained in the treatment of chronic occlusive arterial diseases; treatment of erectile dysfunction; used to protect the mucous membrane of the stomach with a long reception of non-selective inhibitors of cyclooxygenase; in obstetrics, in order to accelerate the ripening of the cervix and induction of birth, etc.

The technical objective is to develop a simple, but at the same time highly effective method of conservative treatment of burns of the conjunctiva.

The specified technical task is solved in that in addition to anti-inflammatory, antibacterial and trophic treatment early after burn additionally once intravenously within 2 hours administered 40 μg of alprostadil, dissolved in 200 ml of physiological solution.

Alprostadil improves microcirculation and peripheral circulation, has vasoprotective action (with the systemic administration causes the relaxation of smooth muscle fibers and has susadora tiraumea action without increasing vascular permeability, helps to improve the elasticity of red blood cells, reduces platelet aggregation and activity of neutrophils, increases fibrinolytic activity of blood), that is, in many stages of pathogenesis of burn disease. Application of alprostadil is of particular importance in the treatment of burns of the eyes, accompanied with critical ischemia of the conjunctiva, the outcome of which, in a large percentage of cases is the development of cicatricial deformities, and ischemia of the conjunctiva region of the limb abruptly slows down and often makes it impossible epithelization of the cornea. All this often necessitates surgical treatment of such patients (how agressively to debride with the plastic of the conjunctiva).

The proposed method is an effective method of treatment of burns of the conjunctiva, which improve the course and outcome of burn disease.

The method is characterized by the following clinical examples:

1. Patient, 38 years old, was admitted in ophthalmology hospital with a diagnosis of chemical burns of the conjunctiva of the III degree, corneal II degree of the left eye (vodka). On the 4th day (since burn) the patient was put alprostadil dose of 40 µg described above. On the next day after the drug has been a considerable reduction of ischemia of the conjunctiva and the acceleration of epithelialization of the cornea, surgical treatment of this patient is not panado ilos, the patient was discharged on the 11th day (since burn).

2. Patient G, 69 years old, was admitted in ophthalmology hospital with a diagnosis of chemical burns of the conjunctiva of the eyelids III degree, the cornea of the I degree of the left eye (vinegar essence). On the 3rd day (after burn) the patient was put alprostadil dose of 40 µg described above. On the next day after the drug has been a considerable reduction of ischemia of the conjunctiva and the acceleration of epithelialization of the cornea, surgical treatment of this patient was not needed, the patient was discharged on the 11th day (since burn).

3. Patient, 27 years old, was admitted in ophthalmology hospital with a diagnosis of chemical burns of the conjunctiva II-III degree, the cornea of I-II degree of the left eye (hydrogen peroxide). On the 4th day (since burn) the patient was put alprostadil dose of 40 µg described above. On the next day after the drug has been a considerable reduction of ischemia of the conjunctiva and the acceleration of epithelialization of the cornea, surgical treatment of this patient was not needed, the patient was discharged 10 days (from time of burn).

The method of treatment of burns of the conjunctiva, including anti-inflammatory, antibacterial and trophic treatment, characterized in that early after burn additionally once intravenous drip for 2 hours to enter 40 µg alprose the Dila, dissolved 200 mg of saline.

The method of preparing the lenses is characterised by that at first lenses are put in partially fluorinated organic compounds, and then in fully fluorinated organic compounds. Fluoroorganic saturation of organic matrix of the soft contact lenses is consistent. At first water is displaced from the lens matrix with one of partially fluorinated organic compounds or their mixture, and then surplus partially fluorinated organic compound or their mixture is removed with completely fluorinated organic compound or their mixture. Water is displaced from the lens matrix until steam bubbling stops. The surplus partially fluorinated organic compound or their mixture are displaced from the lens matrix is ensured by heating the lenses in the medium of completely fluorinated organic compound or their mixture during 10-15 minutes.

EFFECT: invention provides improved service characteristics of the soft contact lenses, increase in oxygen-permeability, and resistance to corrosive mediums that ensures prolonged carrying thereof.

For this purpose intravenously introduced is preparation "Photosens" 0.01-0.1 mg/kg of weight 10-20 hours before laser exposure. Laser exposure is realised on subretinal neovascular membrane with wave length corresponding to peak of absorption of photosensitiser with total light doze 300-500 J/cm2 one time in 24-hours per course of 1-5 sessions. 48 hours after laser exposure medicine which blocks growth factor of vessel endothelium is introduced endovitreally one time.

Method for preparation of medicinal SCL on the basis of polymer hydrogels by means of lens saturation with antibiotics from aqueous solutions, in which SCL are represented by lenses on the basis of cross-linked polymer 2-hydroxyethylmethacrylate, and for lens saturation with antibiotic they use aqueous solution, which contains mixture of antibiotics - fluoroquinolone and cephalosporin with mass ratio of fluoroquinole / cephalosporin from 0.06 to 0.67 and overall concentration of antibiotics in solution from 1.25 to 5.5 wt %. In method of medicinal SCL preparation fluoroquinole is substituted with ofloxacin or levofloxacin, and cephalosporin - with cefotaxim or cefuroxim. As SCL is saturated with fluoroquinole and cephalosporin from solution that contains mixture of antibiotics, sorption of both fluoroquinole and cephalosporin is increased compared to sorption from individual solutions.

EFFECT: invention makes it possible to increase time required to extract fluoroquinole from medicinal SCL 1,4 times, and time required to extract cephalosporin 4 times.

SUBSTANCE: therapy of the patients suffering from chronic lower limb ischemia involves sampling the autogenic blood cell mass in amount 6-8% of total blood volume by discrete plasmapheresis. Then the autogenic cell mass is incubated with pentoxifylline 10 ml during 20 min at room temperature with ATP 2 ml added. The prepared mass is reinfused to the patient. Thereafter, Vasaprostan is introduced in a dose 40 mkg/day with physiologic saline 200 ml as intravenous infusion during 3 hours. Therapeutic course is 10-15 daily procedures.

EFFECT: higher clinical effectiveness with regard to the pathology due to correction of hemodynamic disorders in an ischemic focus and improvement of rheological properties blood in the absence of absolute indications to surgical procedure or inability thereof.

It is ensured by mixing preparation Vasaprostan 20 mkg and human albumin 10% 100 ml. The mixture is incubated in a thermostat within 20 min at temperature 37°C and then slowly introduced to a patient intravenously drop-by-drop within 2-3 hours, once a day. Therapeutic course is 10 days.

EFFECT: incubation of Vasaprostan and albumin in a developed mode provides binding with protein and prolongation of therapeutic effect that in turn allows improving clinical effectiveness with decreasing therapeutic dose of the preparation.

SUBSTANCE: invention concerns medicine, therapy. The way includes blood sampling and its centrifugation. Then allocation of erythrocyte fraction and plasma removal is carried out. An angiotropic medicinal substance in quantity of an average therapeutic dose on each 200 ml of erythrocyte fraction is administered into the allocated fraction. Then irradiation with low-intensity helium-neon laser radiation with 1.2 mW power within 20 minutes is performed. 100 ml of a normal saline solution on each 200 ml is added in erythrocyte fraction and returned within 1.5-2 hours.
