FIELD: medicine.

SUBSTANCE: invention can be used for adjustment of an internal circle of an anatomic aperture or a lumen, in particular at an annuloplasty of cardiac valves. The device for adjustment of an anatomic aperture or lumen internal circle contains the opened ring bound to an agent of the opened ring circle size adjustment. The agent of the opened ring circle size adjustment is executed in the form of the guide tube located in a ring socket and bridged to the case. The adjusting handle with possibility of rotary and longitudinal moving concerning the case is established in the case. One extremity of the opened ring is fixed on a guide tube, and the second extremity executed extended, is placed in the guide tube with possibility of moving along it and fixed on the adjusting handle.

EFFECT: simplification of an internal circle of anatomic aperture or lumen adjustment process, simplification of the device design and increased reliability.

The invention relates to medicine and can be used to regulate the internal circumference of an anatomic orifice or lumen, in particular when annuloplasty heart valves.

Known also support ring for annuloplasty heart valves (see RU patent for the invention №2300348, CL A61F 2/24)containing an armature in the form of a broken ring of super-elastic Nickel-titanium coated xenopericardial treated with epoxy compounds.

The main disadvantage of the known open support ring is that it after some time cease to meet the functional state of the myocardium, resulting in increased load on the heart and reduces the effectiveness of myocardium in the postoperative period. In order to maintain the desired functional state of the myocardium have to make several additional surgical interventions.

Known implantable device for adjusting the internal circumference of an anatomic orifice or lumen (see C. No. 2005108673, EN, CL A61F 2/24), taken as a prototype, containing annular open rim associated with the annular open rim tool that allows you to adjust the circumference of the specified annular rim from the first circle to the second, and means for maintaining the rim in said first or second circle. Tool that allows you to adjust the circumference of the annular rim from the first circle to the second circle contains the teeth formed on the annular rim, a gear mechanism, in the form of a worm gear, a shaft, means for rotating the shaft and means for remote control.

A disadvantage of the known device is the design complexity, lack of reliability and the complexity of the regulation of the internal circumference of an anatomic orifice or lumen.

Objective of the claimed invention is the simplification of the regulatory process internal circumference of an anatomic orifice or lumen, and also simplify the design of the device and improving the reliability of his work.

The problem is solved due to the fact that in the device for controlling the internal circumference of an anatomic orifice or lumen containing the open ring, associated with means for regulating the size of the circle broken rings, means for regulating the size of the circumference of the open ring is made in the form of a guide tube located in the connector ring and connected with the case, which set the adjustment knob with the possibility of rotational and longitudinal movement relative to the housing, with one end open ring closed is captured on the guide tube, and a second end, made elongated placed in the guide tube to move along it and fixed on the adjustment knobs. In addition, the device is provided with a screw mounted in the guide tube with the opportunity to interact with an elongated open end of the ring.

The use of the claimed invention will greatly simplify the design of the device for controlling the internal circumference of an anatomic orifice or lumen, to improve the reliability and efficiency of its work, and also to simplify the procedure repeated changes of broken rings.

The claimed invention is illustrated by the drawing, which shows a General view of the device for controlling the internal circumference of an anatomic orifice or lumen in the section.

The claimed device comprises an open ring 1 and floor 2. The ring 1 is made of synthetic or metallic filaments, for example of the super-elastic Nickel-titanium (patent of Russia №2300348, CL A61F 2/24). The coating can be performed, for example, made of synthetic fabric or knitted materials (U.S. patent No. 6217619, CL A61F 2/24). Floor with both ends attached to the guide tube 3 made of synthetic material, for example, from PVC skin). The upper end of the guide tube 3 is sealed, and the other horse is connected to the housing 4, in which you installed using the threaded connection of the adjusting knob 5. The device operates as follows.

In order to move the adjustment knob 5 in the longitudinal direction relative to the housing 4, it is necessary to rotate. When this occurs, adjust the internal circumference of an anatomic orifice (depending on the direction of movement of the adjusting knob 5 in the housing 4 is the contraction or spread of the ends 6 and 7 open ring 1). For the production of fine adjustment of the dimensions of the open ring adjustment knob 5 is provided with a graduated scale, for example, a dial plate (not shown). After reaching the required size of the open ring 1 its elongated end 7 is fixed by pressing it with a screw 12 to the inner surface of the guide tube 3. Then remove the terminal 10, releasing the elongated end 7 of the connection with an adjustment knob 9, and disconnect the housing 4 with a built-in adjustment knob 5 of the guide tube 3 (unscrewing, if the casing was installed on the guide tube on the thread; cutting the tube, if the end of the guide tube 3 was glued into the housing bore 4, and so on). The freed end of the guide tube 3 is closed tightly (sealed) and under local anesthesia sewn under the skin of the patient. If necessary, re-adjust the dimensions of the open ring 1 is fixed to the end of the insulating tube 9 is removed under local anesthesia from the adipose tissue, cut its sealed end. The guide tube 3 is attached (for example, using thread or glue) case 4 with an adjustment knob 5. By turning the adjusting knob 5 is re-adjusting the size of open ring 1. SUBSTANCE: said valve comprises a ring box and a number of indented leaves to be attached thereto. The ring box is supplied with an opened conductive element arranged therein or on its surface. Fastening indents for each leaf are interconnected in pairs with the conductive element contacting to the conductive element of the ring box. The method involves fitting the valve with a radiator and recording electromagnetic radiation within radio frequency band therefrom, as well as determining the dysfunction by frequency response deviation from the reference received from a standard valve. SUBSTANCE: fragments aorta and pulmonary artery together with a valve unit are isolated from donor heart in sterile conditions. Then isolated homografts are estimated for adaptability. The fragments of aorta and pulmonary artery together with the valve unit are isolated in a sequence as follows: aorta and pulmonary artery are divided to fibrous rings of aortic and pulmonary valves; it is followed with excision of distal parts of pulmonary artery 2 cm away from artery mouths; left and right coronary arteries are isolated from epicardium and cut off 0.7-1 cm away from mouths; right ventricle of heart and interventricular septum are cut off 2-2.5 cm away from pulmonary valve in parallel to coronary transverse from heart edge to interventricular septum; then pulmonary homograft with muscular flange 2-2.5 cm of right ventricle of heart is isolated from fibrous ring, while aortic homograft with a flange 1.5-2 cm is isolated from fibrous ring of valve, herewith mitral valve anterior leaflet is left without chords to be cut off with shortening muscular flanges of transplants up to 0.5 cm in width and thickness. To test the valve unit for adequacy, homografts are turned inside out to make a visual inspection of valves, sinuses and vascular walls.

EFFECT: method allows standardising tissue preparation for transplantation, reducing number of damages of valve unit, isolated homografts, decreasing defect ratio and reduction cost price of commercial homografts, improving functional reliability of a transplant due to removal of models with border changes of valve tissues observed.

SUBSTANCE: heart valve bioprosthesis comprises a flexible bearing frame formed by three axially symmetrical arches joining the rack tops. The commissure tops are formed by shutters connected by sutures and fixed on the bearing frame enclosed in a biocompatible material, e.g. xenopericardium. The enclosure shaped as the top edge frame, while its bottom part is shaped as an edge of fibrous ring of recipient aortic valve and forms an attached cuff. SUBSTANCE: valve contains a frame with a carrier ring corresponding with the natural shape of fibrous ring of human valve, a rack and an obturative shutter element attached to the frame. The carrier ring of the frame is segmented on perimetre, while the rack supports are V-shaped and interfaced to the ends of the specified segments of the carrier ring. The racks are provided with platforms to fix the tips of papillary muscles. The method consists that the carrier ring is segmented by number of racks. The prosthesis contains a ring-shaped case and rotary cusps. The agent of deduction of the cusp in the case is executed in the form of two pairs through grooves forming among themselves two mutually-grasping structures. An adjustable gastric band comprises a flexible inextensible strip connected with an elastic membrane one end of which is connected with a flexible tube, while the other end is closed. Besides, the gastric band accommodates a port attached to an inflatable membrane via the flexible tube and supplied with a membrane to be needled to introduce liquid into the membrane. SUBSTANCE: invention relates to ophthalmosurgery. The accommodating intraocular lens has optical and support elements. The optical element is made in form of a first closed ellipsoidal elastic transparent polymer jacket, filled with transparent liquid. The jacket is symmetrical about the optical axis and can change is geometrical shape and can provide for reciprocating motion of the front surface of the optical element along the optical axis of the accommodating intraocular lens, as well as change of curvature of the peripheral area of the jacket in the space in front of the capsular sack. The support element is made in form of a second closed ellipsoidal elastic transparent polymer jacket, filled with transparent liquid. The jacket is symmetrical about the longitudinal axis of the support element, can change its shape inside the capsular sack and provides for reciprocal motion along the optical axis of the accommodating intraocular lens. The support and optical elements are joined together by an elastic channel and are made such that, the transparent liquid can flow from one jacket to the other.

EFFECT: improvement of accommodating properties, spatial distribution of local deformations of tissue of the capsular sack, reduction of the size of these deformations and reduction of permanent injury of tissue of the capsular sack.

The first support element is made in form of a closed ellipsoidal elastic transparent polymer jacket, congruent to the inner surface of the capsular sack and filled with transparent liquid. The first support element can change its geometrical shape and provides for reciprocal motion along the transverse axis of symmetry, which coincides with the central optical axis of the optical element. The surface of the first support element has, on its side facing the capsulorhexis zone, not less than three rectangular stop lugs, located at equal angular distance from each other and each made in form of a rectangular plate, bent at a right angle. The second support element is made in form of an elliptical segment, arched congruently to the outer surface of the first support element and is provided with not less than three lugs, situated at equal angular distance from each other. The second support element can interact with the stop lugs of the first support element and is joined to an elastic optical element, which can execute reciprocal motion along the transverse axis of symmetry together with the second support element.

EFFECT: improvement of accommodating properties, reduction of local deformations of tissue of the capsular sack.

The optical element of the first intraocular lens is made in form of a segment of a biconvex lens, the chord of which lies horizontally on the line of the optical axis of the intraocular lens. The arc of the chord of the optical element of the first intraocular lens faces downwards with respect to the line of the optical axis of the intraocular lens, and the principal plane of the optical element of the first intraocular lens is parallel the principal plane of the optical element of the second intraocular lens. The optical element of the first intraocular lens is in front of the capsular sack. Support elements of the first intraocular lens are in form of at least, three curvilinear arcs, which interact with a capsular ring installed in the capsulorhexis zone, and is the first part of the support elements of the second intraocular lens, which interact with the second part of the support elements of the second intraocular lens, made in form of at least three arcs joined by one side to the capsular sack, and with the other to the rear surface of the optical part of the second intraocular lens. The arcs are congruent to the shape of the inner surface of the capsular sack, and the rear surface of the optical part of the second intraocular lens is congruent to the rear surface of the capsular sack.

EFFECT: improvement of spatial distribution of local deformations of tissue of the capsular sack, reduction of the size of these deformations, reduction of injury of tissue of the capsular sack and provision for good near and far eyesight.

SUBSTANCE: device has head, neck, trochanteric part, pedicle, axle, elastic members and casings having internal conic surfaces. The axle has variable cross-section with grooves on its surface arranged at an angle. The casings are mounted in tubular part of the pedicle uniformly along its length. The axle is mounted in the casings and longitudinally movable as a result of action of cylindrical portion of the trochanteric part. The elastic members are manufactured as hollow body of arc revolution having longitudinal slits. They have end flanges one of which is rigidly fixed on the axle and the other one is movable and has fingers movable along the grooves. The elastic members embrace the axle on the segments of greater diameter and are engageable with internal conic surfaces.
