Method of treating acute radioreactions in patients with larynx cancer

FIELD: medicine.

SUBSTANCE: invention relates to medicine, oncology, surgery, radiology, physical therapy and can be used for treating patients with larynx cancer. Area of radiation and surrounding tissues is exposed to infrared laser irradiation. From first day of radioreaction diagnostics, exposure to infrared laser irradiation with constant magnetic field with magnetic induction intensity within 20 mTa, recurrence frequency of pulse laser irradiation of infrared spectrum within 80 Hz, power 0.5-1.0 W on all zone of irradiation is performed with defocused ray for 30-120 seconds. Course includes 3-8 procedures with further application on irradiation zones and intake of a teaspoon of cedar nut oil and before going to bed daily during the whole course of radiation therapy.

EFFECT: method allows to increase efficiency of treatment of early radioreactions and prevent development of later radiation damage reducing severity and frequency of their development.

There is a method of treatment of acute radiation reactions, including pharmacotherapy [1]. However, this method is not effective enough, because it recommends only the use of drugs, the use of which is not sufficiently effective and adequate, as it has some side effects, such as allergic reactions, in addition, it is not possible in a short period of time to completely remove radiation reaction, especially in the defeat of the skin. The main effect it aims to mucous membranes and to a lesser extent on the skin.

Closest to the present invention is a method for the treatment of radiation reaction [2] when radiation epithelia oropharyngeal area using a red laser radiation. The scope of it is limited, because the penetrating ability of the radiation spectrum is small (in millimeters), so the depth of the skin, its effect is not fully.

New technical problem - increasing the efficiency of the method by reducing the frequency and severity of radiation reactions, reduction of terms of treatment.

To solve the problem in the treatment of acute radiation reactions in patients with cancer of the larynx include the eating of the effect of infrared laser radiation in the irradiation region and the surrounding tissue, from the first days after diagnosis of daily radiation reaction effect of infrared laser radiation with a constant magnetic field with the intensity of the magnetic induction within 20 MT, a repetition rate of pulsed laser radiation in the infrared spectrum in the range of 80 Hz with 0.5-1.0 W across the exposure zone defocused beam within 30-120 sec, the course includes 3-8 procedures, with subsequent implementation of applications for radiation areas twice a day: immediately after treatment, combined with the ingestion of cedar oil on a teaspoon and night, every day during the entire course of radiation therapy.

In the existing literature is not found the proposed combination of distinctive features and they are not explicitly derived for the expert from the prior art. Thus, the proposed method meets the criteria of the invention of "novelty" and "inventive step. This method has been clinically tested at the Institute of Oncology in conjunction WITH Russian. Thus, the method meets the criterion of "industrial application".

The method is as follows: from the first days after diagnosis of acute radiation reactions are daily exposed to infrared laser radiation with a constant magnetic field, the intensity of the magnetic induction within 20 MT, h is depends repetition pulsed laser radiation in the infrared spectrum in the range of 80 Hz, with a capacity of 0.5-1.0 W across the exposure zone defocused beam for 30-120 seconds. The course includes 3-8 procedures depending on the type of radiation reaction, carried out daily with the subsequent implementation of applications for radiation areas twice a day: immediately after treatment, combined with the ingestion of cedar oil on a teaspoon and at night (only application), daily throughout the course of radiation therapy.

Cedar oil, sertaline handmade, manufactured by LLC "Laboratory of Ruzaeva", Tomsk, Russia THE 9141-001-58913946, has a pronounced anti-inflammatory and wound healing properties.

Example 1.

Patient S., 54 years old, diagnosed with Cancer of the larynx (T3N0M0), followed by radiation therapy. The patient complained of pain in the radiation zone, difficulty swallowing, pain when passing food, cough. Diagnosis: Acute radiation reaction to a dose of 44 Gy - epithelitis, dermatitis. The treatment according to the proposed method: a day in the first days after diagnosis of acute radiation reactions assigned to the influence of magneto laser using a blurred beam of infrared laser 80 Hz, 120 sec. the area of irradiation and the surrounding tissues, with the influence of a constant magnetic field of 20 MT, daily, twice a day: eposredstvenno after the procedure was performed by application of cedar oil on the irradiation zone with simultaneous ingestion teaspoon of cedar oil and at night (only application) before the end of the course of radiation therapy, leading to a radical dose within 8 days. Upon completion of the treatment course of 8 days was observed complete disappearance of pain and restored function of swallowing, stopped coughing. The phenomenon of epithelia and dermatitis were not registered.

Example 2.

Patient P., 58 years old, diagnosed with Cancer of the larynx (T2N0M0). In the preoperative period prescribed course of DHT. Acute radiation reaction to a dose of 20 Gy - epithelia. The patient complained of sore throat, hoarseness and pain when swallowing. The following treatment was performed in accordance with the claimed method, namely: daily was administered the magneto laser defocused beam on an area of exposure, frequency of 80 Hz for 30 seconds on the whole area of the irradiation and the surrounding tissues with the influence of a constant magnetic field of 20 MT, daily for 3 days followed by applications and the simultaneous ingestion of a teaspoon of cedar oil immediately after the procedure and at night in the form of applications throughout the course of radiation therapy, leading to a radical dose. Upon completion of the treatment of complaints and phenomena of epithelia not registered, the voice was restored.

The mode of the proposed method of treatment is chosen on the basis of data analysis of clinical observations. Treatment by the claimed method was performed in 72 patients in the Institute of Oncology in conjunction WITH the AMS. The treatment results were evaluated according to clinical and laboratory studies, as well as perform endoscopic examinations - fibroadipose.

Feature of the proposed method is the possibility of its use in the early period after diagnosis of radiation reactions with damage as mucous membranes, and skin. On the basis of clinical observations of patients with this pathology parameters of mode of exposure and dose of cedar oil selected optimal in obtaining anti-inflammatory and healing effect, aimed at creating conditions for removal of radiation damage and repair the skin and mucous membranes due to the normalization of blood circulation, especially in the area of the small blood vessels of the irradiation zone and surrounding tissues. The proposed regime contributes to shortening the healing time due to the stimulation of trophic and enhance local immunity. In addition, the conditions in preventing the development of late radiation reactions in the remote period. Infrared laser pulse mode, which is the basis of magnetic-laser therapy, avoids thermal effect due to the short pulse, which penetrates deeply into tissue (4-8 cm), which creates the possibility to influence the whole of the mucous membrane and the thickness of skin and subcutaneous clutch the key during the procedure, by changing the distance from the surface of the skin. The presence of the magnetic field and the application of cedar oil, which has anti-inflammatory and wound healing properties, in the form of applications in the area of influence of radiation therapy twice a day - right after the procedure, combined with the simultaneous ingestion and at night in the form of applications, enhance this effect, and to prevent the development of radiation fibrosis. Dose of oil - one teaspoon is optimal for healing and reduce inflammation in the affected area. Hyposensitizing these physical factors allows you to remove and prevent the development of allergic reactions. The conditions for more effective healing of the surface on which the manufactured radiation exposure, and cosmetic treatment of the skin, which leads to prevention of the formation of fibrous connective tissue and the development of late radiation reactions.

Thus, the proposed method has several advantages: reduces the frequency of side effects during radiation therapy, creating the opportunity to continue when necessary courses of radiation therapy, leading to a radical dose. The application of the method prevents the development of late radiation reactions, reduces the recovery time in patients undergoing radiation therapy and creates prob is the possibility to continue continuously required a course of radiation therapy, improving the quality of treatment and duration of life for patients.

Said therapy is ensured by introduction of cisplatin and leucovorin, and continuous intravenous infusions of fluorouracil dosed 300 mg/m2. Cisplatin solution is introduced intravenously drop-by-drop and dosed 6 mg/m2 daily once a day within 1 hour. Leucovorin is injected intravenously jet-like and dosed 30 mg/m2.
