Method for diagnosing intestinal helminthiases

FIELD: medicine.

SUBSTANCE: method involves carrying out irrigoscopy examination under artificial hypotonia of large intestine with single-stage double opacification of large intestine done with fine peptized aqueous barium cocktail heated to 36°C composed of water - 100 ml, sodium citrate - 1.5 g, sugar - 8 g, barium sulfate - 200 g, carboxymethyl cellulose 0.5 g, raw hen eggs - 2 eggs. Irrigoscopy examination is repeatedly carried out in 24 h. Ribbon-like shadows being observed, intestinal heminthiasis is to be diagnosed.

EFFECT: high reliability in determining shape and localization of helminth even in the cases of negative coprological and serological examination results.

The invention relates to medicine, namely to gastroenterology, Parasitology and radiology, and may, in particular, be used for the diagnosis of the three classes of worms: round, tapeworms and flukes.

From the practice of medicine is known for the traditional methods of diagnosing helminthiasis, as laproscopically and serology.

However, the known methods have significant drawbacks. Diagnostic value of these methods is low because of insufficient specificity and sensitivity, as well as the frequent absence of worms and their eggs in fecal material. In the early stage of the disease in the absence of adult worms, but also when the invasions caused by larval forms, parasitological diagnosis based on coproscopy, it is extremely difficult and often impossible (Ozeretskovskaya N.N., Zelnova NS, Topolska NI Clinic and treatment of helminthiasis. - Moscow. - 1985. - P.19-21). On the territory of Russia is dominated by low-intensity invasion, especially when geohelminth, so if coproscopy have to use concentration methods (methods of Fulleborn, Kalantaryan), which further complicates the methodology (Menshikov V.V., Delektorskaya, L.N., Zolotnitskaya R.P. and other Laboratory methods in the clinic. - Moscow. Medicine, 1987. - S).

Well-known and widely used such STRs is about the diagnosis of parasitic infection of the gastrointestinal tract, as a serological study with specific antigens.

However, the known method has a number of disadvantages. First of all, the high cost and lack of information, high frequency of false-positive reactions. In addition, it is not possible to determine the localization of the helminth.

The aim of the invention is to improve the efficiency of diagnosis of parasitic infection of the large intestine.

Put in the invention the objective is achieved by the fact that they have a barium enema in conditions of artificial hypotonia of the colon with the use of single-stage double-contrast study of the colon, carried out by the introduction of warmed up to 36°With fine pepsirefresh water-barium cocktail, having the following composition: water - 100 ml, sodium citrate 1.5 g, sugar 8 g, barium sulfate 200 g, carboxymethylcellulose, 0.5 g, raw chicken eggs - 2 pieces; after the entry into force of this cocktail and holding over after 24 hours, re-perform x-rays of the colon and when it detects ribbon-like shadows diagnose intestinal helminthiasis.

The inventive method for the diagnosis of parasitic infection of the large intestine is new and are not described in literature.

The proposed method achieved the following positive effects: improving the efficiency of diagnosis of Helmi Tazov large intestine, save energy and money. The method is simple to perform, requires no special equipment and expensive medicines. The method provides high reliability to determine the form and location of the helminth. Moreover, it is often effective even in case of negative coprological and serological studies. The method is based on detection of ribbon-like prosvetleni in the colon or body of helminth when the absorption patinirovannoy, radiopaque barium suspension. The developed method is recommended when identifying representatives of the three classes of worms: round, tapeworms and flukes.

The proposed method is as follows.

After careful preparation of the patient for the examination is carried out simultaneously double contrast study of the colon (APC), the method is carried out according to the MOH orders No. 129 and No. 132. Must use original fine pepsirefresh water-barium cocktail and artificial obstructions of the colon. Relaxation of the colon is achieved with medication (2-3 tablets Aeron or 100 mg pinaverium bromide (dicetel) 15-20 minutes prior to the study). Contrast water-barium cocktail has the following composition: water - 100 ml, sodium citrate 1.5 g, sugar 8 g, with lift barium 200 g carboxymethylcellulose - 0-5 g, raw chicken eggs - 2 pieces Blend is prepared on an electric mixer for 5-6 minutes. In one study enough 150-200 ml. To optimize the research process and better absorption contrast cocktail worms it is recommended to warm up to 36°C.

After conducting over to visualization of worms needed after 24 hours, re-x-ray of the large intestine. To confirm and analysis ribbonlike shadows are targeted x-rays on film format 24×30 cm with subsequent inspection of the abdominal cavity in 24 hours.

The proposed method was tested on 520 patients who were treated in various offices and clinic AM design Bureau No. 1 Astrakhan from 1999 to 2004. Below are the results of testing.

A survey of 520 patients. Indications for radiographic examination for the presence of helminths were: anaemia unclear geneza, recurrent allergies, irritable bowel syndrome, chronic colitis, partial intestinal obstruction, eosinophilia, subfebrility unclear etiology. In parallel, all patients were conducted laproscopically study (feces on eggs of worms). In 162 cases were visualized tape is visible shadows in the lumen of the large intestine, characteristic of worms, with only 48 patients the diagnosis was confirmed when aimed triple laproscopically study, 15 patients during endoscopic investigations and surgical interventions.

As an illustration, the following clinical observations:

Example 1. Patient B., 51. Enrolled in the surgical Department of the AM design Bureau No. 1 with complaints of persistent constipation, weight loss, weakness, fatigue, malaise. On examination, attention, severe pallor of the skin. To clarify the causes of anemia and partial intestinal obstruction the patient hold the barium enema in conditions of artificial hypotonia of the colon with the use of single-stage double-contrast study of the colon, carried out by the introduction of warmed up to 36°With fine pepsirefresh water-barium cocktail, having the following composition: water - 100 ml, sodium citrate 1.5 g, sugar 8 g, barium sulfate 200 g, carboxymethylcellulose, 0.5 g, raw chicken eggs - 2 pieces barium enema: in the splenic angle is determined by the large size (3x6 cm) exophytic education with rough contours covering the entire lumen intestine with superstartickets.com extension. After the entry into force of this cocktail and holding over 24 hours again about the W ill result x-rays of the colon: in the splenic corner and descending part of the colon is determined by many ribbonlike shadows.

Conclusion: the obstruction of the colon caused by a conglomerate of worms.

Example No. 2. Patient M., 24 years. For a long time suffers recurrent allergoses. At another hospitalization in the pulmonary branch of the AM design Bureau No. 1 revealed anemia of mixed Genesis (12 and iron deficiency). Conducted in-depth examination of a patient, comprising the subsequent barium enema examination of the colon after 24 hours (see example 1). In the left parts of the colon revealed ribbonlike shadows characteristic of worms. Conclusion: Helminthiasis.

Example No. 3. Patient S., 38 years. For 4 years she was on "D", accounting for about iron deficiency anemia. She passed an in-depth examination, including barium enema performed on a traditional three-step methodology. Causes of anemia remained unclear. At the next hospital Hematology Department at AM design Bureau No. 1 held x-ray examination of the large intestine by the proposed method. In the transverse colon, splenic and descending sections of the colon revealed multiple ribbon-like shadows. Conclusion: Helminthiasis.

Example No. 4. Patient K., 43 years. Was admitted to the gastroenterology Department of the AM design Bureau No. 1 with complaints of pains in the paraumbilical area, frequent, loose stools mixed with blood. With the purpose of specification of the diagnosis item is avadana barium enema (APC) with the screening of the colon within 24 hours. In the elongated sigmoid and transverse colon revealed many ribbonlike shadows. Conclusion: Helminthiasis.

Example No. 5. Patient C., 36 years. For a long time suffered from chronic constipation. During the examination in the clinic at the place of residence revealed iron deficiency anemia. The last 5 years, has not been barium enema or colonopenbracket. The survey was conducted in the clinic AM design Bureau No. 1. Barium enema (APC) by the proposed method: in splenic corner and descending part of the colon revealed many contrasting ribbon-like shadows. Conclusion: Helminthiasis.

Analysis of clinical testing shows that our proposed method of radiation imaging of intestinal helminthiasis, based on the holding over under artificial hypotonia of the colon with the use of single-stage double-contrast study of the colon, carried out by the introduction of warmed up to 36°With fine pepsirefresh water-barium cocktail, having the following composition: water - 100 ml, sodium citrate 1.5 g, sugar 8 g, barium sulfate 200 g, carboxymethylcellulose, 0.5 g, raw chicken eggs - 2 pieces; after the entry into force of this cocktail and holding over after 24 hours, re-perform fluoroscopy colon and upon detection of ribbon-like shadows diagnose intestinal the helminthiasis; significantly expands the diagnostic capabilities of the Clinician, opens new perspectives for conducting etiologic therapy.
