Distraction apparatus for forming infantile hand's pollex

FIELD: medical equipment.

SUBSTANCE: apparatus intends for surgical cure of children with inborn or acquired defects of pollex. Distraction apparatus has oval ring with holes, needles and profiled bracket with two lobes, provided with holes for needles. Bracket is fastened to distraction bar by nuts. Distraction bar passes through support plank, which is made to have working surface's slope of 25°. Plank is fixed in holes of oval ring by means of two bars and nuts to provide movement along those bars. Needles are fixed in holes of oval ring and lobes by means of screws. Distraction bar has flat side surface to match the hole in support plank for preventing rotation.

EFFECT: elongation of III middle metacarpal bone and proximal phalanx of pollex; adjustment of speed of distraction; reduced weight and size of construction.

The invention relates to medical technology, is intended for the surgical treatment of children with congenital and acquired defects of the fingers.

The closest analogue prototype is a distraction apparatus for lengthening I metacarpal bones of the hand (Vladimir folks O.V. Apparatus for lengthening I metacarpal bones of the hand. The invention and judging. H 2. M. 1979), used to restore the first finger of the hand. The apparatus consists of a ring-oval and semi-interconnected three distraction rods, spokes and clamping devices.

The disadvantage of the above device is that the ring and the semicircle apparatus of large diameter, rounded forms are in the same plane without taking into account the physiological deviations I finger brush that does not allow elongation of the Metacarpals and the main phalanx of the first finger of the hand in the desired plane. The locking elements of the apparatus involve the use of standard spokes diameter of 1 mm or more. Therefore, it is not possible to use thinner needles in children of younger age. The unit itself has a significant size and weight that does not allow you to use it in children of younger age.

The task of lengthening the metacarpal bones and the main phalanx of the first finger of the hand in the desired plane, the individual adjustment of the rate of distraction and led the ranks of the extension segment of the finger, weight reduction and simplification of the structure.

The task is achieved using the proposed distraction apparatus for the formation of the first children's finger brush (figure 1).

The drawing shows a schematic representation of the distraction apparatus for the formation of the first children's finger brush. Distraction apparatus has two types of size oval rings and support bars for the younger and older age. Distraction apparatus consists of an oval ring 1 with the holes 20 and spokes 17, curly brackets with two petals 11 having holes for spokes 12, which is fixed to the distraction rod 10 with nuts 14, passing through the support bar 4, the latter performed with the inclination of the working surface 9 of 25° and mounted in the holes 20 of the oval ring 1 by means of two rods 2, 3 and nuts 5, 6 can be moved along the rods, spokes 17, 19 are mounted in the holes oval rings 20 and petals curly brackets 11 by means of screws 13, 18, and the distraction rod 10 has a flat side surface corresponding to the opening 15 in the support plate 4 to prevent rotation.

Curly bracket 11 is fixed rigidly nuts 14 to distraction rod 10 with a flat side surface and is movable to a shape of the hole 15 on the working surface 9 of the support strip 4 by means of nuts 16. The presence of the flattened edges of the openings 15 is and the working surface 9 of the support strip 4 and a flat side surface on the distraction rod 10 eliminates the rotation moment extendable fragment at the time of turning a wrench when the distraction. Use spokes of a standard sample with a diameter of 0.6-0.8 mm Screws (4 mm diameter) have a wide head with a slot on top (for screwing with a screwdriver) and a through hole with a diameter of 1.0 mm under the head for the passage of the spokes. They do not bear their design features. Holes in the oval ring is made with a diameter of 4.2 mm under used screws for fixation pins and rods. Rod for fixing the support bracket with a threaded throughout with a diameter of 4 mm also do not have their own design features.

Distraction apparatus for the formation of the first finger brushes are used as follows.

Spend two spokes 17 through the proximal divisions I, II, III, IV, V metacarpal bones of the hand. The spokes 17 are fixed in the holes 20 of the oval ring 1 of the apparatus by means of screws 18. Then through the distal section I Metacarpals spend two intersecting spokes 19, which is fixed in the curly bracket 11 through the opening 12 at an angle of 30-45° by means of screws 13. After fixing apparatus in brushes make a linear incision on the back beam edge of the stump of the first finger length up to 2 inches, subperiosteal naked I metacarpal bone (the main phalanx of the first finger) and produce a transverse osteotomy. Then the wound is closed with two interrupted sutures.

By dosed rotation of the nuts 16 on the distraction rod 10 with curly distraction with the Wallpaper 11 relative to the support strip 4 is realized by lengthening I metacarpal bone (the main phalanx of the first finger). In the process of forming regenerate I metacarpal bone (the main phalanx of the first finger brushes carry out an individual regulation of the rate of distraction and dimension of elongation of a segment of a finger.

The advantages of the proposed design:

- reduce the weight and size of the apparatus by reducing the number of parts (one of the distraction rod), the thickness and diameter of the oval ring, replace the ring on the support bar and the locking bolts for the spokes on the screws smaller in relation to the prototype;

- the use of apparatus in younger children, because there are two sizes designs and needles have a diameter of 0.6-0.8 mm;

- the ability to lengthen metacarpal bone in the desired plane as the support strap has a slope of working surface 25°;

- no rotary effect, because of the distraction rod has a flat side surface of the corresponding hole in the support strip;

- ability to prevent blocking of the tendons of the fingers and damage to vessels and nerves due to fixation of the finger in curly distraction bracket consisting of two lobes rounded with holes for holding the spokes at an angle of 30-45°;

- possibility of selecting the desired view when conducting x-rays to monitor the formation of the regenerate I Metacarpals (cos the main phalanx of the first finger), since bone does not overlap the metal parts of the console.

Distraction apparatus for the formation of the first children's finger brush containing oval ring with holes and spokes, characterized in that it has a curly bracket with two petals, with holes for the spokes, which are secured to the distraction rod nuts passing through the support bar, the latter is tilted work surface 25° and mounted in the holes oval ring by means of two rods and nuts can travel on these rods, spokes are fastened in the holes oval rings and petals by means of screws, and the distraction rod has a flat side surface of the corresponding hole in the support strip for prevent rotation.

Middle supporting members are manufactured as semi-rings having five slots four of which serve for receiving threaded rods and one of them for receiving holder screw. Fracture zone hydrotreated by introducing Novocain solution mixed with demineralized bone matrix and primary clavus osteoperforation is done before reposition to prevent hand articulation from contracture taking place. Device has supporting member, special-purpose fixing members, wire bundles and fixing nuts. The supporting member has proximal and removable distal parts manufactured as treaded rods connected to each other by means of connection member having casing and threaded axial canal for receiving a treaded rod and canal for receiving wires. SUBSTANCE: apparatus has spring-loaded branches connected with one another through pivotal lock with shaft and mating opening. Arms and working jaws are provided on branches.
