Device for correcting funnel chest deformities

FIELD: medical engineering.

SUBSTANCE: device has two supporting rectangular platforms having rounded edges and rectangular plate. Lateral surfaces are rounded each having single hole for fastening the platforms. Each of the supporting platforms have two holes at the ends and are hold in the plate hole with fixing member on one side and with rivet on the other side. Supporting platform having fixing member is removable.

EFFECT: prevented mediastinum organs injuries.

The invention relates to medicine, namely to surgery, and is intended for the correction of funnel chest deformity.

There are several devices for the correction of funnel chest deformity[2, 3, 4, 5, 6].

A device for correction of funnel chest, curved shape of the thorax [1].

The disadvantage of this device is that the ends of the device, while the intercostal spaces, are attached to the adjacent edges of the threads. Load them when the correction is large, which leads to the disappearance of the device into the chest cavity, contributing to relapse funnel chest deformity.

The proposed device for the correction of funnel chest, is made in the form of a plate, wherein the plate is made in the shape of a rectangle, the side surface of which is rounded and have one hole, and it rivets fasten two rounded rectangular reference sites, which have two holes at the ends, with one of the sites removable. A platform mounted clamp on one side and a stud on the other.

A device for correction of funnel chest consists of a plate 1 and two reference PL is of venues - 2.

The edge of the main plate 1 are rounded and have the edges of the holes 3. Edge of the reference sites too rounded and have two holes 4. Supporting pad 2 is attached to the plate 1 through the Central hole 3. On the one hand rivet, with another latch 5.

A device for correction of funnel chest deformity works as follows.

Under General anesthesia is semi-oval cut length of 3 centimeters in the area of xiphoid process and opened the retrosternal space, separated adhesions are exempt parietal pleura on the right and left liners to line. Below the areola on liners line on both sides at the level of the fifth or sixth intercostal space are parallel 3 cm sections and through the intercostal space forming a tunnel in the chest, through which is conducted the edge of the plate 1 without the supporting point 2, under the control of a finger, allowing you to control and prevent damage to the organs of the mediastinum. After conducting plate 1 with a given configurata and correction of funnel-shaped deformation in the hole 3 of the main plate 1 is inserted into the support platform 2 with the latch 5 and placed on two adjacent edges, to which the supporting pad 2 is attached by threads. Wounds sutured in layers. The operation lasts 30-40 minutes, blood loss during this operation, the minimum and transfusion cruiseline not need. Two days later, the patients begin to walk. 7-8 day remove the seams and 10-12 day are discharged home.

The condition is satisfactory. Active. If the load appears shortness of breath. The skin and the regular color, clean. Breathing well is held on both sides of the net. The number of respiratory movements alone - 20; load 28-30 per minute.

Muffled heart sounds, rhythmic, pulse 92 / min. Blood pressure is 85/60 mm Hg.

Abdomen without features. Physiological functions are normal.

Locally: sternum sinks along with the ribs, starting from the second rib and below. Depth zapadenia 2,5-2,8 see Costal arch deployed. The deformation is symmetric. With a deep breath retraction increases.

On radiographs of the chest in two projections from 16.04.2004 year from lung and heart no changes. Index deepening of 0.64.

Blood from 16.04.2004 year: hemoglobin - 104 g/l; erythrocytes - 3,2·1012g/l; platelet - 418·109g/l; leucocytes 12·109g/l; segmenta Ernie 54%; eosinophils - 5%; lymphocytes - 39%; monocytes - 2%. ESR - 8 mm/hour. Protein - 56,6 g/l; urea 3.2 mmol/l; glucose - 4.5 mmol/l

ECG dated 15.04.2004 year: sinus rhythm, heart rate of 95 per minute, the tendency to bradycardia. The electrical axis of the heart is not rejected. Incomplete blockade of the right bundle branch. Diffuse changes in the myocardium.

Diagnosis: congenital symmetrical funnel-shaped thorax deformation of the II degree.

21.04.2004, the child was operated. Made correction of funnel chest deformity metal plate 1 with the supporting platforms 2.

22.04.2004 he was transferred from the emergency Department. Began to walk. The postoperative period was smooth.

28.04.2004 year the stitches removed, and on the same day discharged home.

SUBSTANCE: at the bottom of spinous process and posterior part of its arch in frontal plane one should form a canal, resect caudal part of spinous process and semi-arch of the above-located vertebra so to form two transplants upon connecting flaps to replace the defect of inter-articular parts of arch after dissecting the obtained osseous fragment and inter-spinous ligament. Then on should introduce a rod into this canal and move it cranially to fix the ends in screws' caputs inserted into the roots of vertebral arches in case of spondylolysis that enables to restore the function of vertebral mobile segment.

SUBSTANCE: the present innovation deals with corporectomy in patients with pathological vertebral fractures. One should introduce an insertion into inter-body defect after pre-manufacturing it out of cement as a groove of 2-3 mm thickness to be placed into defect with its protuberance towards dural sac at 2-3 mm against it; in case of a great inter-body defect one should introduce drainages from the side of safe vertebrae with possibility for their removal. If additional fixation is necessary one should introduce metal rods into vertebrae being adjacent with inter-body defect which partially protrude into defect area that prevents neural structures against thermal and mechanic damage.

SUBSTANCE: device has rod having bendable end and threaded end with nut and sliding union thrusting against cortical plate of fractured bone segment. The rod has sharp tip round part to be introduced into the bone fragments by rotating and four-sided part to be introduced by hammering. SUBSTANCE: intraosteal retainer has pin with cavity and two slots at the end of pin, and two wires positioned in pin for further accommodation in slots. Distal ends of two wires are equipped with round loops. Intraosteal retainer is provided with compressing unit comprising spring, supporting plate with wire receiving openings, and nuts. Proximal ends of wires are provided with thread for nuts. Proximal ends of wires are disposed in spring and supporting plate. Cavity in pin is formed opened along the entire length. The clamp has irregularly curved supraosseous member and straight pedicles fixable in bone and inclined toward each other. SUBSTANCE: one should cross femoral adductors in the site of proximal fixation, resect a skin fragment out of ischial bone for the length of 0.5-1.5 cm being above ischial tuber. SUBSTANCE: method involves making recess in upper end of femur diaphysis in sagittal direction. The recess is brought in engagement with external roof edge of cotyloid cavity roof. The lower extremity is moved aside to at an angle of 45°, foot is fixed in neutral position and rehabilitation measures are done. Bed rest with the lower extremity moved aside to at an angle of 45° and foot fixed in neutral position, is administered during the first 10 days after surgical operation. Medical training exercises comprise isometric exercises done with extremity muscles and passive-active flexures in knee joint within 5 min 3 times a day every day. Sitting in bed is allowed beginning from the third day. Healthy leg is placed on a chair when sitting in bed in early postoperative period from tenth to twentieth day. Medical training exercises are done three times a day during 10-15 min. Dorsal muscles are strengthened in inclining and straightening the body to 50-100 times per day. Active flexure loading over operated lower extremity knee joint is added beginning from twentieth to thirtieth day and the patient starts actively moving the straightened leg upward from the bed to 100 times per day and sitting in bed with both legs got down drawing the treated leg aside. The number of active flexures is increased in knee joint making the straightened leg move upward from the bed to 200 times per day from the thirtieth to fiftieth day in later period and the patient starts mastering active leg deflection aside and bending it in the hip joint done 10-15 times 3 times a day. The patient stands up and retains standing position by means of crutches near the bed drawing the operated extremity 30-35° aside from the thirtieth to fiftieth day in later postoperative period. Leg shortening is compensated with shoe heeltap. The patient sits down and stands up from chair him/herself 10 times three times a day and masters walking on crutches deviating leg to 30-35° 15-30 min long 3 times a day loading the operated leg not higher than 15% of full load in outdoor treatment mode 3 months long beginning from the fortieth day. Isometric medical exercises training lower extremity muscles are done to 500 times per day with a leg drawn 25-30° aside from lying position, making efforts to turn the leg off from bed plane to 100 exercises 3 times a day. Active operated leg flexure in knee joint are done to 100 movements 3 times a day. Bending and straightening body in lying on the bed doing to 100 exercises a day. Actively drawing the operated leg aside in standing to 100 movements 3 times a day. Active operated leg flexures in hip and knee joint are done in standing to 100 movements 3 times a day. Walking on crutches is carried out not longer than 1,5-2 h long a day leaning on operated leg drawn 30-35° aside having heel-tapped shoes on for compensating leg shortening. Load applied to the operated leg is increased by 10-15% every month after being dismissed from hospital when compared to norm. Walking time is increased by 30 min. The angle the leg is drawn aside is reduced to 15-20°. Cane is used in walking and heel-tap is selected to suitable height. The extremity is brought to functionally right position at the sixth month after being dismissed from hospital. The heel-tap is reduced to required height for adjusting real anatomical shortening. Walking with cane and medical training exercises are continued. Massage and swimming is practiced.

EFFECT: enhanced effectiveness in recovering physiologically correct locomotor function in the hip joint.

SUBSTANCE: one should isolate and dissect the tendons of anterior tibial and long fibular muscles to dissect the tendon of posterior tibial muscle against the site of fixation and direct it towards plantar rear area in front of internal ankle through the tunnel in subcutaneous fiber, then one should subcutaneously direct dissected tendons of anterior tibial and long fibular muscles onto plantar rear area to suture them so to leave free the tendinous end of posterior tibial muscle which should be intraosseously fixed to median wedge bone that keeps the process of foot repulsion during walking.
