Method for predicting pathology in development of pulmonary and aortic cardiac valves in male neonatals

SUBSTANCE: in male neonatals one should measure intra-operational values for the parameters of pulmonary (P1) and aortic (P2) cardiac valves and calculate C coefficient according to empirical formula: C = ψP1/P2, where C - empirical coefficient being equal to 1.95, and if C values are beyond 1.36-3.12 interval it is possible to conclude upon pathology in development of aortic and pulmonary cardiac valves. The innovation suggested enables to perform quick-test diagnostics of valvular pathology in neonatals.

EFFECT: higher accuracy and efficiency of diagnostics.

The invention relates to the fields of cardiology and cardiac surgery.

There is a method of setting parameters of the heart, described in the work of the Rowlatt U.F., Rimoldi H.J.A., Lev M. The quantitative anatomy of the normal child's heart" (Pediatr. Clin. North. Amer. 1963; 10: 499-588), which consists in measuring the parameters of the heart in children.

However, this work has significant drawbacks: it does not give sufficiently accurate diagnostic effect due to the fact that no account was taken of the floor, and changing the true parameters of the heart due to pre-fixing of the studied drugs formalin solution.

The technical problem solved by the invention is improving the accuracy of disease diagnosis development of pulmonary and aortic heart valves in the newborn male.

The technical problem is solved in that in the method for the diagnosis of pathology of the development of pulmonary and aortic heart valves in the newborn male, which consists in measuring the parameters of the human body, additionally measured intraoperative values of the perimeters of the lung (P1) and aortic (P2valves of the heart, and then determine the coefficient of proportionality development (K) by the empirical formula

The proposed method is as follows. Initially measure intraoperative values of the perimeters of the lung (P1) and aortic (P2) heart valves. Intraoperative measurement of perimeters valves are manufactured with the help of the device, the design of which is "know-how".

Then determine the coefficient of proportionality development (K) by the empirical formula

Developed by the authors of the essential distinguishing features is necessary and sufficient for an unambiguous positive solution to the stated technical problem of improving the accuracy of diagnosing pathology of the development of pulmonary and aortic heart valves in the newborn male.

Authors conducted long-term studies allowed us to empirically determine the value of the coefficient Ψ.

The proposed method is characterized by the following clinical examples.

Example 1.

Patient M., male, age - 17 days. Measure intraoperatively the perimeter of the pulmonary valve (P1), which is 22 mm. Then Meryem intraoperative perimeter of the aortic valve (P 2), which is equal to 19 mm, the Coefficient of proportionality development (K), defined by the formula above, is the value of 2.26. Thus the value of K lies in the range of from about 1.36 to 3.12.

Thus diagnosed normative development of the pulmonary and aortic valves of the heart in a newborn baby boy age 17 days.

Example 2.

Patient I., male, age 1 month. Measure intraoperatively the perimeter of the pulmonary valve (P1), which is equal to 31 mm Next measure intraoperatively perimeter of the aortic valve (P2), which is equal to 19 mm, the Coefficient of proportionality development (K), defined by the formula above, is the value 3,18. Thus the value of K lies outside the interval from about 1.36 to 3.12.

Pathology in the development of the pulmonary and aortic valves of the heart in a newborn baby boy age 1 month.

Example 3.

Patient K., male, age 20 days. Measure intraoperatively the perimeter of the pulmonary valve (P1), which is equal to 20 mm, Then measure intraoperatively perimeter of the aortic valve (P2), which is equal to 12 mm, the Coefficient of proportionality development (K), defined by the formula above, is the value of 3.25. Thus the value of K lies outside the interval from about 1.36 to 3.12.

Pathology in the development of pulmonary and aortalnah the heart valves have a newborn baby boy age 20 days.

Example 4.

Patient S., male, age 5 days. Measure intraoperatively the perimeter of the pulmonary valve (P1), which is equal to 25 mm Further measure intraoperatively perimeter of the aortic valve (P2), which is 22 mm, the Coefficient of proportionality development (K), defined by the formula above, is the value of 2.21. Thus the value of K lies in the range of from about 1.36 to 3.12.

Thus diagnosed normative development of the pulmonary and aortic valves of the heart in a newborn baby boy age 5 days.

Example 5.

Patient C., male, age 8 days. Measure intraoperatively the perimeter of the pulmonary valve (P1), which is equal to 26 mm Next measure intraoperatively perimeter of the aortic valve (P2), which is equal to 23 mm, the Coefficient of proportionality development (K), defined by the formula above, is the value of 2.2. Thus the value of K lies in the range of from about 1.36 to 3.12.

Thus diagnosed normative development of the pulmonary and aortic valves of the heart in a newborn baby boy age 8 days.

The use of this invention in the practice of the Scientific Center of cardiovascular surgery named. After Bakulev RAMS allowed us to unambiguously prove a positive solution to the stated technical problem - improving the accuracy of diagnostics the pathology of the development of pulmonary and aortic heart valves in the newborn male.

SUBSTANCE: as functional parameters one should detect the amplitude for pulse oscillations, the period of motor wave and the value for tissue oxygenation. Moreover, one should measure the amplitude of pulse oscillations and the value of tissue oxygenation in tested section during the period of 1 motor wave, not less. At values of one and more amplitudes of pulse oscillations being below 2 mm and the value for tissue oxygenation being below 80% on should conclude upon disorders in viability of the section under testing. In case of no motor system in tested section one should simultaneously detect the period of motor wave in adjacent intact section. SUBSTANCE: the present innovation deals with surgical treatment of lower limbs ischemia at considerable lesion of distal channel and impossibility to conduct direct revascularization of vascular channel of affected limb. One should perform mechanical perforation of bony tissue up to medullary canal in epiphyses of femoral and leg bones being 4-5 cm, correspondingly, higher and lower against a knee joint, and, also, in the lower third of shin, metatarsal bones and phalanges followed by stimulation of bone marrow and tunnelization of muscular tissue of femur and shin by applying perforation foramens upon femur and punctures in additionally performed at lateral and posterior surfaces of shin by performing, moreover, per three tunnels from each foramen and puncture in.
