Method for restoring 2/3 of crown part defect of frontal teeth groups with wire network reinforcement pin being applied

FIELD: medical engineering.

SUBSTANCE: method involves placing wire network reinforcement pin in composite material medium applicable for restoring crown tooth part to enable solid connection of retained tooth part and one under restoration as well as additional restored tooth reinforcement to take place. The pin has U-shaped part for reinforcing crown tooth part under restoration and a part used for fixing it that is manufactured as a bundle of free longitudinal wire ends of metal net and surface counterpart surface of the cavity formed in base of tooth root and retained crown tooth part.

EFFECT: high strength under chewing loads applied; retained anatomical natural tooth shape and appearance.

 

The invention relates to medicine, namely to the dentist, and can be used in the restoration of 2/3 of the defect coronal part of the front groups of teeth with the use of the grid-wire reinforcing pin.

The level of technology

From public sources of information on the priority date for the application is known that the causes of defects in the coronal part of the frontal teeth groups can be: caries, non-carious lesions, trauma to the tooth, high abrasion, traumatic otom (1-3).

To restore the defects of the coronal part of the front teeth groups in clinical practice, the use of pins and copings made from different materials, with subsequent restoration of artificial crowns.

Recently, in order to restore the defects of the coronal tooth structure is widely used direct and indirect methods restorations using light-cured composite materials, advanced reinforced glass ionomers cements, composites, chemical curing and a variety of pin structures (4-9).

The closest in technical essence is taken as a prototype method to repair a damaged tooth with a pin having a head with extended area for fixation of a tooth crown (10).

The disadvantages of this method are:

1)the need for laboratory stages;

2) difficult and time-consuming procedure of manufacture of the pin;

3) the presence of defects in the casting of the pin in its manufacture, resulting in excessive tooth preparation;

4) excessive expansion of the root canal for installation of the pin, resulting in reduced mechanical strength of the root;

5) excessive load on the root due to an effect of a "wedge" in the apical part of the tooth under load;

6) the possibility of corrosion of the pin, which can lead to fracture of the tooth followed by its loss;

7) the duration of treatment.

The objective of the invention is to develop a means of restoration 2/3 defect coronal part of the front groups of teeth with the use of the grid-wire reinforcement dowel pin, ensuring elimination of the above drawbacks, while maintaining the high aesthetics of the restored tooth.

Effect: increase the strength of the missing coronal tooth structure through the use of simple in the manufacture of wire and wire pin while ensuring appropriate structural, functional and aesthetic characteristics of a restored crown of the tooth.

The technical result is due to the design of the pin used in the restoration of the tooth crown, and the sequence of steps the procedure of restoration. Grid-wire pin provides a strong connection restored and preserved parts of the tooth, and also reinforces the restored tooth by placing the pin in the environment of the composite.

In particular, the technical result is ensured by the following factors:

1) due to the execution of a pin made of metal mesh in the form of monolithic mechanically robust construction, located in the layer of composite material with a view to further reinforcing the restored tooth;

2) due to the execution of a pin made of metal mesh, resulting in increased area of contact with the composite, reduced shrinkage of the composite material, improved marginal seal of composite to the remainder of the tooth hard tissues;

3) due to the execution of a pin made of metal mesh, resulting in a decrease of pressure (stress), which occurs during polymerization;

4) due to the use as starting material for the manufacture of the pin, metal mesh, reducing the likelihood of complications, chips and bruised after the restoration, leading to increased operational period of the restored tooth;

5) due to the use as starting material for the manufacture of the pin, metal mesh, providing high aesthetic properties is and restored tooth;

6) through the use of a metal grid as a starting material for the manufacture of the pin, this allows for the configuration of the pin directly in the implementation of the method of restoration of the tooth crown;

7) a simple method of manufacture of the pin provides a low cost of manufacture, allows the restoration of the defect of the tooth in one visit.

The invention

The proposed method includes pre-endodontic treatment of the tooth. The root canal should be filled about 1/3 of the length of the root and not have periapical changes.

Determines the color of the future restoration on the VITA scale. Is retraction of the gingival retraction thread for more access to the cervical part of the tooth crown. After gingival retraction on the remaining coronal tooth structure and on the basis of the root is formed cavity.

Then carry out the next step of the method of tooth restoration: the making of a rectangular metal grid grid-wire pin, whereby repaired using composite materials coronal part of the tooth is firmly fastened to a surviving part of the tooth and additionally reinforced.

The width of the rectangular metal grid must be 2-4 mm larger than the width of the defect of the tooth crown. To the th grid along the length of the bend at a right angle, departing at 1-2 mm, giving it a U-shaped profile. The width of the formed U-shaped profile should match the width of the defect of the tooth crown. When the fixing pin in the root canal and cavity formed in the preserved tooth, the pin is positioned so that the curved edge of the U-shaped profile facing towards the vestibular surface of the tooth. Forming a U-shaped profile allows you to increase the strength of the pin bending, and the placement of the pin increases the bending strength of the restored tooth.

Next, form the part of the pin, whereby it is fixed in the root canal of the tooth and in the previously formed cavity.

To this end, one of the ends of the U-shaped profile curved edges closer to each other, after removing from the grid portion of the transverse wires. The u-shaped profile, thus, becomes a “bunch” of free ends of the longitudinal wires. The length of the “beam” of the free ends of the wires should be no more than 1/2 the length of the root canal of the tooth. And the surface of the transition from the U-shaped profile to the “beam” of the free ends of the longitudinal wires are formed with mating surfaces of a cavity formed in the base of the tooth root and its remaining coronal part.

After fabrication of the pin engaged its fitting and cor is ctyroky.

Prepared for the next phase of restoration of the coronal part of the tooth mesh-wire pin consists of:

fixing the part intended for fixation in the root canal of the tooth and in the cavity formed in the base of the root and the remaining coronal tooth structure,

and a reinforcing part in the form of a U-shaped profile, which is intended for reinforcement of restoration of the tooth crown.

After fitting and correction grid-wire reinforcing pin produce acid etching the surface of the cavity formed in the hard tissues of the tooth crown and the root, and the root canal is closed with cotton wool. Acid wash running water while running the saliva ejector, air dried and applied the adhesive, lightly blow air to evenly distribute, perform a standard polymerization method according to the instructions. Next, the wool is removed from the root canal and the grid wire reinforcing pin is fixed in the root canal using stacionarnogo cement.

After fixing the grid-wire reinforcing pin in the channel of the tooth cavity with the fixed pin fill in layers of composite material. Then proceed to the final restoration of the missing crown of the tooth with composite materials in accordance with its anatomical shape./p>

To achieve the aesthetic effect of process and grinding restored tooth standard polishing disks of different thickness and grain size of the abrasive and diamond forests.
