The invention relates to medicine, namely to traumatology. The invention provides a stable compression of bone fragments without damaging pisarei zone of the radial bone. The plate consists of invoice parts and has holes for screws. The invention relates to surgery, in particular for traumatology and orthopedics for the treatment of epiphyseolysis and osteophytosis of the radial bone in children and adolescents.

However, this plate requires two to three screws through the epiphysis that the child can destroy the cartilage of the epiphysis, damage Psarou zone (area of growth). The use of T-shaped plate is not possible to achieve stable compression of bone fragments. Installing this plate is combined with a wide dissection of the capsule of the wrist joint.

The objective of the invention is a stable compression of bone fragments without damaging pisarei zone of the radial bone by fixation of bone fragments with a special plate.

The task is implemented as follows. For accurate mapping of fragments of the radial bone, stable fixation and compression of bone fragments without damaging pisarei zone (zone of growth) using a special plate for osteosynthesis of epiphyseolysis and osteophytosis of the radial bone in children, which consists of slip portion made in the form of a rectangle, one end of which made two of stiletto, and have holes for screws. The plate has two sizes, corresponding to ages 7-11 and 12-15 years:

for 7-11 years - slip part of the plate length of 30 mm, thickness of 1.5 mm, a width of 9 mm (Fig.1, POS.1) has the stilettos on one edge, angled 70to the plane of the plate, is applied to the epiphysis and directed to its center. The length of the stylets 8 mm, thickness 1.5 mm, width 2 mm, their ends sharpened, the distance between the stylets 5 mm (Fig.1, POS.2). The proposed plate osteosynthesis epiphyseolysis and osteophytosis of the radial bone in children is used as follows. After selecting the place of fracture of the radial bone and the mapping of its fragments in the epiphysis introduced the stilettos plate (Fig.2, POS.1). After that, through the oval hole of the plate near the proximal edge of the hole twist cortical screw (Fig.2, POS.2) to its full length, thus achieving compression of bone fragments. The next step twist cortical screw (Fig.3, POS.1) through the round hole of the plate, thus achieving rigid fixation of the radial bone fragments.

The advantages of plate osteosynthesis epiphyseolysis and osteophytosis of the radial bone in children:

- eliminates the possibility of damage to the distal zone of the growth of the radial bone;

- sufficient in the process of post-operative treatment;

- reduced time external immobilization, as compressicauda stability of fragments allows to reduce them and to start early functional rehabilitation treatment;

- eliminated the possibility of damage to the articular cartilage of the wrist joint.

Example 1. Vladimir 3., 10 years (IB No. 4119), was admitted to children's trauma Department of the CST, Khabarovsk No. 27.08.01 diagnosed with a closed distal osteophytosis right radial bone with displacement of fragments; closed fracture of the right elbow bone in the lower third shift fragments; private Razmyslov fracture of the right humerus with displacement of fragments. Under General anesthesia produced by the operation - open reposition and fixation of the fragments of the radial bone plate osteosynthesis epiphyseolysis and osteophytosis of the radial bone in children. The postoperative period was uneventful wound healing came by primary intention. The child was discharged from the hospital on the 14th day outpatient treatment of trauma. Viewed through 1.5 months after surgery: no complaints, range of motion in the wrist joint is complete, the anatomical axis of the limb is correct. Radiographically: fusion of fragments of the radial bone, sootnoshenie who stepped in child trauma hospital №2, Khabarovsk 20.08.01 diagnosed with a closed distal osteophytosis left radial bone with displacement of fragments. Under General anesthesia produced by the operation - open reposition and fixation of the fragments of the radial bone plate osteosynthesis epiphyseolysis and osteophytosis of the radial bone. The postoperative period was uneventful wound healing came by primary intention. The child was discharged for outpatient treatment on the 17th day after the operation. Examined 2 months after surgery: no complaints, range of motion in the wrist joint is complete, the anatomical axis of the limb is correct. Radiographically: the complete fusion of fragments of the radial bone, the ratio of the bone anatomy of the wrist joint is correct.

SUBSTANCE: one should fulfill two vertical mucosal incisions of transitional fold being below and laterally against infraorbital foramen. Then, according to these incisions, one should form foramens in anterior wall of maxillary antrum, dissect cicatricial adhesions of soft tissues in area of restored osseous fragments at isolating periorbital fiber till its free movement together with oculomotor muscle in orbital cavity. One should forma fatty interlayer between oculomotor muscle and osseous wall, moreover, a transplant should be fixed from one side towards anterior department of the lower orbital wall, and from another side - to osseous department of retrobulbar orbital part. Then it is necessary to suture osseous fragments of orbital wall with the transplant along the whole length. The innovation enables to form correct ratio between osseous fragments of periorbital fiber and oculomotor muscle, decrease traumatism of mucous-periosteal layer in area of vestibule of mouth, decrease disorders in circulation in area of operation and, also, reconstruct normal movement of patient's eyeball.
