The way dynamic roentgenomammography

The invention relates to medicine and can be used when performing dynamic roentgenomammography. The study is being conducted in the sitting position of the patient. On the same x-ray film produced three x-ray of the rectum with the capture of the coccyx and the pubic joint is at rest, and with a straining effort made by the patient volitional muscle contraction locking apparatus. The method allows to increase the value of the examination of the status and function of the rectum and pelvic floor muscles, to reduce radiation exposure to the patient and medical staff. 1 Il.

1. Description of the invention

a. The prior art (the prototype of the method and its criticism)

The prototype of the invention is a Method of radiological examination of the rectum” (priority reference No. 96112725, from 16.09.1996). This method consists in the following.

X-ray examination of the colon is carried out in conditions of x-ray television. It is performed after the preliminary preparation of the colon laxatives and cleansing enemas or after lavage gastrointestinal belong on the first workplace x-ray machine.

The large intestine is filled with barium suspension as normal over are in the horizontal position of the patient. After filling out the guts of the table apparatus is translated into a vertical position. Run two x-ray (2430) rectum lateral projection with the capture of the coccyx and the pubic bones alone and with a straining effort. Then the patient is asked emptied in the toilet and then in a vertical position are two periapical radiographs of the rectum in the lateral projection, at rest and during natureway. After that the table is translated into a horizontal position. The colon is filled with air, barium enema is performed in conditions of double contrast study.

In patients with disorders of the rectum, this method allows to estimate the emptying of the distal colon, the position of the anorectal area relative to the pubic-coccygeal line, abnormal mobility of the rectum, the presence of a rectocele, intussusception, of rectal prolapse. However, this method does not allow to evaluate the function separatelynikon apparatus of the rectum, the state of the pelvic floor muscles, it is not always possible to identify the abnormal mobility of the rectum (normal mobility Kish is their state of the rectum. Additionally, the method associated with significant radiation exposure to organs of the small pelvis of the patient.

B. The invention

Preparing the patient for the study and preparation of contrasting suspension is the same as in the prototype. The research is being done on the ground the workplace x-ray machine. After filling the colon barium suspension table of the apparatus is transferred to the vertical position. The study is performed with the patient sitting, because this provision is more physiological. On the same x-ray film (see the drawing), without displacement of the object of research, produce three side picture of the rectum, with the capture of the coccyx and the pubic joint is at rest (2), with straining effort (3) and produced by the patient's volitional contraction mouse locking device (4); however, significantly reduce physical and technical parameters of the x-ray. After this, the patient is asked emptied in the toilet, Desk translate in a horizontal position, the colon is filled with air and barium enema is performed in conditions of double contrast study. An assessment of the intestine is relatively h-coccygeal line (1), which is held between the coccyx (5) and the pubic articulation.

This MCI; additional arbitrary contraction locking apparatus provides an opportunity to assess the condition and function of the pelvic floor muscles, the degree of compensation of their disorders, the availability of pathological mobility of the rectum (normal motility of the colon with natureway). Since the proposed method is carried out in more physiological conditions, in comparison with the prototype, significantly increases its informational content. In addition, this method significantly reduces radiation exposure to organs of the small pelvis of the patient (50% - from 15 to 7.5 mSv), which is consistent with the order of Ministry of Health of the RF “On the reduction of radiation exposure to the population.”

Thus, the information completeness of the study, which is very important for diagnosis and differential diagnosis of diseases of the distal colon. The proposed method extends the capabilities of the x-ray method, not complicating it, reducing the total time of the study and significantly reducing radiation exposure to the patient and medical staff.

, Information confirming the possibility of invention

This method is applied in the SSC of proctology is in with a diagnosis of rectocele” of varying severity, 6 - with a dropped crotch and 4 - with internal rectal intussusception. In 20 patients with rectocele study performed twice: before and after surgery and objective control.

Claims

The way dynamic roentgenomammography, which consists in contrasting distal colon barium suspension, the examination of the patient in an upright position and with a straining effort, characterized in that the research is being done with additional volitional contraction locking apparatus, while performing all three images on a single x-ray film, without displacement of the object of research.

FIELD: medicine, cardio-vascular surgery.

SUBSTANCE: one should perform clinical and angiographic testing in two projections. By angiograms one should detect the types of aortal filling defects that form aortal coarctation. One should evaluate isolated aortal coarctation by its angiographic types which should be classified based upon combination of the types of aortal filling defects in two projections. Depending upon the type of aortal coarctation one should perform its catheter balloon angioplasty, stenting of affected area or surgical correction of aortal coarctation. The method enables to match adequate surgical treatment.

EFFECT: higher efficiency of diagnostics.
