(57) Abstract:

The invention relates to medicine, namely to ophthalmology, and can be used to restore vision in severe Belm. Keratopathies contains temporary liner and removable optical cylinder, provided with an external thread. The support part is movable and consists of fitted internally threaded sleeve and placed crosswise extending beyond the perimeter of the cornea of the fixation plates. The sleeve is made in one-piece and provided with a trapezoidal arranged crosswise and transversely to the longitudinal axis of the sleeve slits. The slots serve for attachment of the fixation plates. The fixation plate is made as a separate trapezoidal extended on one end of the tape. Parameters details keratoprosthesis selected in accordance with the parameters of the operated eye. The technical result of the invention is to increase reliability of fixation of keratoprosthesis in the eye by increasing the diameter of the optical part, which in turn prevents the possibility of rejection keratoprosthesis. 3 Il.

The invention relates to medicine, namely to ophthalmology, and is intended for prosthetic cornea at t the second part, made in the form of a ring with frames (see and.with. N 506964, M. CL. 4 A 61 F 2/14, BI, 1985, NO. 43).

The disadvantages of the above technical solutions include nereshennosti basic parts, limiting the area of fixing the limits of the cornea, which does not allow to increase the diameter of the optical part with a sufficient degree of fixation, preventing rejection.

The closest to the technical nature of the claimed createprocessw is a technical solution (patent RF N 97120604/14 from 10.12.97, priority from 10.12.97, a positive decision on the grant of a patent from 11.02.99), in which keratopathies consists of a collapsible inner and outer bearing parts, temporary liner and removable optical cylinder, provided with an external thread, while the folding of the supporting part is made in the form provided with the internal thread and the locking ring is split sleeve, and placed crosswise extending beyond the perimeter of the cornea associated with parts of the sleeve fixation plates.

The disadvantage of this technical solution are: the relative difficulty of Assembly structures, slowing the progress of the operation; the presence of vertical cracks in the joints of the parts of the bushing, reducing the integrity of the fixation plates, limiting the choice of materials for their manufacture, and precluding the possibility of replacing the plates when they are damaged during the operation.

The technical problem to be solved in the invention, a warning of possible exclusion of keratoprosthesis when increasing the diameter of the optical part.

The solution of the technical problem the invention, representing keratopathies containing a temporary liner and removable optical cylinder, provided with an external thread, and a collapsible supporting part, consisting of provided with the internal thread of the sleeve and placed crosswise extending beyond the perimeter of the cornea of the fixation plates is ensured by the fact that the sleeve is made in one-piece and provided with a trapezoidal arranged crosswise and transversely to the longitudinal axis of the sleeve slots serving for fastening the fixation plate and the fixation plate is made as a separate trapezoidal extended on one end of the tape.

The invention is illustrated by drawings, where Fig. 1 shows the projection of keratoprosthesis in the eye, front view, Fig. 2 is a view of keratoprosthesis in the profile section with a projection in the eye, Fig. 3 shows the junction of the sleeve and fixing the operating plate, 4 - the cornea, 5 - corneal incision, 6 - wheel, 7 - sclera, 8 - conjunctiva, 9 - conjunctival incisions, 10 - direct oculomotor muscles, 11 - flat portion of the ciliary body, 12 is the outer boundary of the flat part of the ciliary body, 13, 14 and 15 ways to secure the distal ends of the fixing plates.

The claimed keratopathies made of the permitted use of biologically inert materials, and the parameters of parts of keratoprosthesis chosen in accordance with the parameters of the operated eye.

Implanted claimed keratopathies in two stages.

The positive effects of the proposed keratoprosthesis are:

- simplifying the construction, reducing the duration and invasiveness of the surgery;

- tightness of the sleeve that improve the postoperative period;

- the possibility of replacing the fixation plates during the operation, if they are damaged;

the design flexibility of the supporting part, which allows the use of the fixation plate made of different materials (hard perforated, elastic porous, elastic);

- saving options for replacing the optical cylinder, access to the inside of the eye, the implantation of the prosthesis in a single step, run antiglaucoma operations in safe areas of the limb.

Thus, in the result of the combination of all these private benefits, which are provided for the possibility of rejection keratoprosthesis when increasing the diameter of the optical part.

The orbital implant is optionally shaped as ball or rounded pyramid.

SUBSTANCE: device is manufactured as monolith disk-shaped diaphragm from flexible material. It has optical and haptic part. The haptic one has colored ring having internal diameter of 3.0-4.5 mm and external diameter of 9.0-10.0 mm, and supporting arc-shaped closed members as vertices of a figure symmetrically or asymmetrically arranged relative to pentagonal star rotation axis within a circle of 11.0-13.5 mm large diameter. To make point touch with surrounding tissues, the supporting members have rounded or sharp ends. Method involves applying the supporting members of haptic part for fixing iridolenticular diaphragm depending on whether lens bursa, capsule or iris residues are available or unavailable in the eye. The iridolenticular diaphragm is implanted via small tunnel incision in the cornea or sclera. Capsule bursa residue being available in the eye, supporting members of the iridolenticular diaphragm under implantation are leaned against the residues without additionally fixing them with sutures and thrust into the ciliary sulcus. Iris residues being available in meridians, closed ends of the iridolenticular diaphragm haptic part supporting members are preliminarily cut off and the supporting members are fixed in the meridians immediately to the iris in clips-like manner. No iris residues being available, the members are additionally sutured to sclera in one or two points. Intact capsule bursa being available, iridolenticular diaphragm is implanted after having completely cut-off the supporting members of the haptic part. Complete or partial lack of capsule or iris support being the case, iridolenticular diaphragm is implanted with suture fixation to sclera being used after having in advance built three sclera pockets in meridians corresponding to the supporting members arrangement in the haptic part of the iridolenticular diaphragm.

EFFECT: stable position and reliable diaphragm fixation; reduced risk of pre- and postoperative complications.

SUBSTANCE: the present innovation deals with creating safe and efficient heteroplastic material for treating progressing myopia, stabilization of dry maculodystrophy and manufacturing antiglaucomatous drainages at maximal decrease of antigenic properties and guaranteed sterility of biomaterial that provides the absence of post-operational complications (chemosis, tenonitis, pridocyclitis) at operative therapy of progressing myopia and guarantees stabilization of myopia progressing, the stabilization of pathological processes at dry maculodystrophy, normalization of intra-ocular pressure at operative treatment of glaucoma at applying the drainage out of the material suggested. In the course of the method in question one should treat cattle pericardium after mechanical purification with 10%-ammonia solution for about 4-5 h, washed with distilled water followed by a 4-fold freezing at -10-12° C and defrosting at +40-45° C, for 7.5 h it should be treated with 6%-hydrogen peroxide solution at +15° C, after cutting the material it should be treated with ultrasound to repeat the whole procedure at mixing device, then material should be dehydrated in alcohols of initial concentration ranged 30-70 volumetric percentage to be applied into the vials with 70%-ethanol and sterilized with ionizing radiation at the dosage of 1.5 Mrad.

EFFECT: higher efficiency.

SUBSTANCE: artificial crystalline lens can be used for additional correction of ametropia due to introduction of changes in focal power of artificial crystalline lens. Artificial crystalline lens has optical and haptic parts. Haptic part is made in form of disc provided with hole in its center. Disc is provided with at least two through grooves. Optical part is placed in center of haptic part. Main optical plane of optical part is disposed higher than plane where the haptic part is disposed. Optical part communicates with haptic one by means of at least two arc-shaped support members, simultaneously bent at plane being perpendicular to main plane of optic part for interaction with through grooves.

Any lens is provided with two diametrically opposed protrusions. Optic part is connected with haptic one. Haptic part is made of two curvilinear spiral members; both bent in one direction, and of fixing member in form of recesses and protrusions disposed inside haptic part. First pat is made in form of two support members, tightly connected in one point. Any member is made in form of arc which changes into open spiral. Recess is made onto internal part of arc. Ends of spirals are oriented in opposite to each other. Second part of haptic part has to be a ring which has at least one recess onto its external surface. The recess is made for interaction with internal recess onto internal surface of arc. Internal surface of ring is tightly connected with edge surface of at least one lens.
