Method for manufacturing partial removable nylon denture with dento-gingival dental clasps

FIELD: medicine.

SUBSTANCE: stage of preparing a cast model from the teeth for re-producing, within the abutment teeth at the gingival margin, a denture wax plate 1.8-2 mm thick is laid so as to make it adjacent to the neck area and the alveolar tissue to fit clasp borders. Its surface is shaped, and a smooth taper is created from the neck area towards the mucous membrane, thereby forming the highest plate thickness within the neck area, and narrowing it down gradually along the alveolar tissue.

EFFECT: preventing gingival mucosal injuries within the abutment tooth, reducing the adaptation length and creating the more comfortable prosthesis use environment.

The invention relates to medicine, namely to orthopedic dentistry, and can be used to improve the efficiency of orthopedic treatment of patients with partial tooth loss when using nylon dentures.

Currently, there is considerable interest in the production of dentures made of nylon material, due to physical and mechanical properties of these materials, which expand the aesthetic and functionality of orthopedic treatment of dentition defects (Trezubov V. V., Kosenko G. A. Qualitative characteristics of the removable plate prosthesis with thermoplastic bases // Institute of dentistry. - 2011. - No. 1. - P. 58-59).

A method for manufacturing removable partial nylon prosthesis with locking elements in the form of periodontal clasps that are part of the denture base, consist of dental and alveolar parts, directly crossing each other, and adjacent to the surface of the mucous membrane of the alveolar process (see I. D. Tregubov et al. The use of thermoplastic materials in dentistry. - M., 2007, p. 53-68).

The main advantage of nylon periodontal clamp is high aesthetics, which is achieved through the properties of the material and design of clamp, �livellara part of which is located on the alveolar portion of the jaw in the area of abutment tooth and comes to the gingival margin, same as the configuration of the alveolar portion and directly adjacent to the mucosa, and dental part goes from the edge of the gums and builds on the cervical part of the tooth, whereby the clasp is virtually invisible, even in the aesthetically important area (Rubinko S. S. et al. Selection of abutment teeth with dentures removable partial dentures // Problems of dentistry. - 2012. - No. 4. - Pp. 72-73).

A method for manufacturing removable partial nylon prosthesis with gingival clasps is the prototype of the offering.

The essence of the known method consists in the following. First get the anatomical impression, which is made of super plasters model. Further on the model mark the boundaries of the prosthesis, prepare it for duplication, produce after the duplication process and cast model of super plasters class IV. On the model-duplicate also note the border of the denture and it made wax composition. In the simulation of the periodontal clamp, softened plate of wax is applied on the model in the area of the abutment tooth, compresses it and cut along the marked boundaries (Fig. 1). Further, the model gypsum in a special cuvette, set the Gating system, and then pour the second part of the cell and digested wax. Next, the cuvette is placed in the injection apparatus and provocatuers of thermoplastics. After the process is complete, the cell is opened, produce processing, polishing of the prosthesis, and the imposition of a nylon denture in the oral cavity.

The drawing is presented in Fig. 1, shows a cross section of the plaster model-duplicate abutment tooth, where:

As can be seen from the drawing of Fig. 1, the clamp design, manufactured in a known manner, such that the alveolar part of the clamp (3b) same as the configuration of the alveolar portion of the jaw (2) in the area of the abutment tooth and directly adjacent to the mucosa of the gums, and the level of dental support portion (3A) is located in the cervical part of the tooth (4A) adjacent to the gingival edge (4B).

The disadvantage of this method is that partial removable nylon periodontal prosthesis with clasps manufactured by this method has a traumatic effect on the mucous membrane of the gums in the area of the abutment teeth that occurs during the operation, the result of structural alveolar part clamp same as the configuration alveolar�of the jaw in the area of the abutment tooth adjacent to the mucosa of the gums, and the level of support the dental portion of the clamp is located in the cervical part of the tooth adjacent to the gingival edge. Because in the process of functioning of the prosthesis performs microensure in the vertical direction, due to the susceptibility of the mucosa, that is, its capacity for passive vertical mobility. Compliance value of the mucosa in different parts ranging from 0.5 mm to 1.9 mm (E. I. Gavrilov, I. M. Oxman prosthetic dentistry. - M.: Medicine, 1978. - 464 p.).

The problem to be solved by the invention is the prevention of traumatic exposure partial removable nylon periodontal prosthesis with clasps on the mucous membrane of the gums in the area of the abutment teeth.

Technical result achieved when implementing the proposed method, is that the level of support the dental portion of the clamp is shifted towards the equator of the tooth, and the alveolar part of the clamp is deflected outward and extends accordingly the amount of displacement of the dental portion of the clamp, whereby under the periodontal clasp is formed a free space, the biggest of which is located in the cervical region of the tooth and gradually comes to "no" towards the mucosa of the alveolar part.

To solve the Crystall�the auditors of the task and the specified technical result at the stage of preparing the plaster model for duplication, in the area of the gingival margin of the abutment teeth, where in the future there will be the dentogingival clasps, paving standard base plate wax thickness of 1.8-2 mm. decrease in the thickness of base plate wax is unable to prevent trauma to the mucosa of the gums in the area of the abutment tooth, and the increase in thickness leads to an excessive protrusion of the alveolar portion of the periodontal clamp in the direction of the soft tissues of the oral cavity, which causes discomfort in the operation of the prosthesis. The method is illustrated by drawings Fig. 2 and Fig. 3.

A method of manufacturing a partially removable nylon prosthesis with gingival clasps is as follows. Get anatomical impression, which is made of super plasters model. On the model-duplicate mark the boundaries of the prosthesis and it made wax composition. In the simulation of the periodontal clamp, structural level dental support portion of the clamp is shifted towards the equator of the tooth (4), by the amount of artificial overstatement of the level of the gingival margin (6) and, as a result, the alveolar portion of the clamp is extended by a corresponding amount and is deflected outward by the amount of artificial thickening in the area of the alveolar portion of the jaw (7), which under the clasp is formed a free space. The subsequent stages of manufacture of the prosthesis does not differ from the known method.

The advantage of this method of manufacture of removable partial nylon prosthesis with gingival clasps is that under the periodontal by Klammer free space is formed, which contributes to the prevention of injury to the mucous membrane of the gums in the area of the abutment tooth. This has reduced the time to adapt and are more comfortable conditions of use of the prosthesis, thereby increasing the efficiency of orthopedic treatment of patients with the use of a nylon denture�V.

A method of manufacturing a partially removable nylon periodontal prosthesis with clasps, incorporating obtaining anatomical impression, the fabrication of a plaster working model, preparation of plaster models to duplication, redundancy, obtaining a plaster model of a duplicate, manufacture wax design of the prosthesis, replacement of wax on thermoplastic, characterized in that at the stage of preparing the plaster model for duplication in the area of the abutment teeth, gingival margin, paving the base plate wax, thickness of 1.8-2 mm, so that was adjacent to the cervical part of the tooth and the alveolar part of the jaw, respectively, the limits of the clamp, create a smooth bevel from the cervical part of the tooth down to the mucosa, forming the greatest thickness of the plate in the area of the cervical part of the tooth and gradually reducing it to "no", the alveolar portion of the jaw.

SUBSTANCE: invention refers to medicine, namely to dental orthopaedics, and aims at analysing the strength properties of temporary fixed dentures. A device for making identical experimental constructions of temporary fixed dentures consists of two mated rectangular box-shaped units and a stamp. The box-shaped units are made of a low-melting metal; an inner surface of each unit has a profile of a negative image of the construction of the temporary fixed denture longitudinally, and triangular saw cuts extending from occlusion surfaces of the negative image of each crown denture passing to the end surface of the box-shaped units. The inner surfaces of the box-shaped units have retention guides in the form of triangular spines on one unit and in the form of triangular recesses on the other one. SUBSTANCE: rigid splint recording of the lower jaw position in relation to the upper jaw is prepared on a denture of one of the jaws. A surface of the recording splint is adjusted in relation to the denture surface of the other jaw. The patient is adapted to the optimum position of the jaws for at least two months to achieve a symmetrical position of heads of the maxillotemporal joints. The jaw casts are mounted in a regulated articulator with the use of an occlusion splint recording the jaw position to be thereafter removed, and a space for mounting prosthetic structures is formed. The optimum position of the heads of the maxillotemporal joints is determined by multispiral computed tomography findings, at least twice. For the first time - prior to the prosthodontic treatment, for the second time and further - upon completion of optimising the lower jaw position in relation to the upper jaw, and further - after the temporary fixation of the prosthetic structures in the patient's oral cavity.

EFFECT: method enables creating the optimum positions of the heads of the lower jaws in the maxillotemporal joints by using an occlusion splint as the maximum occlusion recorder.
