FIELD: medicine.

SUBSTANCE: invention refers to medicine, more specifically to dressing used for closure and treatment of burns, wounds of various aetiology, trophic ulcers, decubitus ulcers, etc. What is described is a bandage in which three components are used: a wound-facing nontraumatic textile layer with antiseptic butole in the concentration 0.05 mg/cm2 to 1.0 mg/cm2 in a therapeutic layer which is coated with a replaceable sorptive layer made of a nonwoven fabric and providing sorption power of the bandage within 15 g/g and 10 g/g once changed. The replaceable sorptive layer is fixed between the therapeutic and sorptive layers by means of an adhesive frame which is a third component, e.g. made of a nonwoven fabric 0.1-0.5 mm thick and 6.0-10.0 mm wide; both surfaces of the frame are coated with a therapeutic adhesive, and an outer size of the framework is equal to a length and a width of the therapeutic layer. The therapeutic layer additionally contains drugs of haemostatic, anaesthetising, antioxidant and/or wound healing action.

EFFECT: production of the sustained-action bandage with extended spectrum of antimicrobial action, improved antimicrobial activity and higher sorption power.

The invention relates to medicine, specifically to dressing tools used for closing and treatment of burns, wounds of various etiologies, trophic ulcers, bedsores, etc.

The effectiveness of the dressing means for closing and treatment of various traumatic injuries, wounds and burns, is mainly determined by their ability to prevent the development of infection and to absorb Raney exudate. In addition, therapeutic effectiveness of dressings increases if it provides other types of therapeutic action (analgesia, prevention of inflammation and accelerates healing).

Known medical bandage, Inadyn (Inadine, directory, Systagenix Wound Management, manufacturing company Johnson & Johnson, USA)consisting of woven viscose fabric impregnated with a solution of povidone iodine. The disadvantage of this armband is the low sorption (not more than 2 g/g), relatively low antimicrobial activity and the absence of prolonged therapeutic action (change bandages are changed 2 times per day).

These disadvantages are partially eliminated in the adopted for the prototype dressing material "Al" with prolonged therapeutic effect, which are used antiseptics chlorhexidine and/or furagin (RF Patent No. 2101033, class A61L 15/22, 1996). "Al has a higher sorption capacity of up to 4.5 g/g is prolonged therapeutic effect - up to 72 hours. However, these antiseptics have a limited spectrum of antimicrobial action, and sorption ability of the dressing material is insufficient, particularly for wounds and burns in the stage of hydration.

An object of the invention is a bandage with prolonged therapeutic effect for the treatment of burns and wounds of various etiologies spread spectrum antimicrobial action, increased microbial activity and with increased sorption capacity.

The problem is solved three-component design bandage consisting of a therapeutic layer of the sorption layer and located between the adhesive fixing of the frame. The design of the bandage is illustrated by a drawing. In the stated bandage as an antiseptic is used Butel, not previously used in bandaging material (RF Patent No. 2115418, class A61K 31/47, 31/71, 1998) containing therapeutic layer is from 0.05 to 1.0 mg/cm2. In therapeutic composition can be optionally included hemostatic drugs (for example, iron gluconate, cuprofen, aminocaproic acid), anesthetic (eg, lidocaine, dikain, amylocaine or trimekain), antioxidant (for example α-tocopherol, dihydroquercetin or rutin) or wound-healing actions (for example, eucalyptus oil, sea buckthorn oil, hydroxyapatite and chondroitin sulfate), providing additional therapeutic effect of the claimed bandage

Increased sorption capacity provided by the fact that on top of the curative layer facing the wound, is exchangeable sorption layer (3 in the drawing)made of needle-punched nonwoven hlopkovolokna paintings, which provides increased sorption ability of the bandage to 15 g/year Additional increase in sorption capacity with long-term use of bandages is achieved by replacing the sorption with the HH.

To this end between the treatment and sorption layer is adhesive frame (2 - a in the drawing), which holds the sorption layer and provides its fixation with the change made, for example, of non-woven fabric with a thickness of 0.1-0.5 mm and a width of 6-10 mm. On both sides of frame coated with the layer of adhesive, for example, medical adhesive used in the manufacture of medical adhesives. The choice of thickness of the adhesive frame is determined by the fact that the blade thickness less than 0.1 mm does not have adequate mechanical strength, and when the thickness of the nonwoven cloth over 0.5 mm deteriorates the contact between the treatment and sorption layers, which leads to a decrease in the sorption capacity of the dressing. When the width of the frame is less than 6.0 mm decreases the reliability of fixing the sorption layer, and when the width of the frame more than 10 mm reduced its effective area.

The presence of the proposed medical bandage effective antiseptic prevents the development of infection and thereby promotes healing of burns and wounds. Holes in therapeutic layer provide free outflow of the fluid, while the sorption layer can be replaced on demand without damaging the wound surface. Suggested dressing has a high degree of surface modeling RAS, easy to use. The test results of the proposed medical dressings in comparison with analog (Inadie) and prototype (antimicrobial cloth of knitted fabric, sterile "Al-HF" with chlorhexidine and furagin) is given in Table 1. Presented in Table 1 data on the sorption ability of the claimed dressing correspond to its use without changing the sorption layer. Described is gel, representing sparsely-cross-linked chitosan and polyanionic hydrocolloid polymer, which has 2-3 linkages of copolymers per chitosan molecule, and distributed in it auxiliary substances. As polyanionic hydrocolloid, used is suspension of sucralfate or polyalginic acid particles, containing 0.01-15 wt % of stabiliser. As stabiliser used is highly molecular polyalcohols, such as polyvinyl alcohol, polyethylene glycol, as well as polyvinylpyrrolidone or silver microparticles. Auxiliary substances are introduced into gel individually or together in concentrations from 0.01 to 30% of gel dry weight. Gel is obtained by mixing chitosan with polyanionic hydrocolloid, into which preliminarily introduced were stabiliser and auxiliary substances. Bandage material represents hydrogel layer, applied on biocompatible film from water-insoluble polymer with pores with diameter D=(0.01-5.0) mcm. Covering can be on wounds during long term from 2 to 20 days and produce medicinal effect during said period.

EFFECT: bandage means can be used in treatment of trophic ulcers and various wound types, including wounds polluted with microorganisms, trophic wounds and prolonged non-healing wounds, and also be applied as allogen skin substitute in operations aimed at recovery of skin in patients with extensive burns.
